The subject of the contract is: Delivery of surgical and treatment gloves:
The order consists of 2 parts:
Part 1: Non-sterile treatment gloves.
Part 2: Surgical gloves, sterile.
1. The detailed description of the subject of the order and the terms of execution
a) Offer form with attachments no. 1.2 - 1.3.
b) Agreement, the template of which is attached as Appendix No. 2 to the Terms of Reference;
c) GDPR information clause constituting Appendix No. 3 to the Terms of Reference.
2. The subject of the order should be performed in accordance with the applicable Polish Standards and other legal regulations regulating the requirements in the field of ergonomics, safety and appropriate quality.
3. The subject matter of the contract must be approved for marketing and use in accordance with the regulations applicable to hospitals. The Contractor is obliged to execute the order on the terms and conditions described in the Terms of Reference and the offer.
