1. The subject of the contract is civil liability, property and motor insurance SPWSZ in Szczecin.

2. The Contractor is obliged to fulfil the order under the terms and conditions described in the draft contract, annex No 4A and/or 4B to the SWZ and in the description of the subject of the contract, as set out in Annex 1 to the SWZ.

3. Local Vision: The Contracting Authority does not provide for an obligatory local vision. The Contracting Authority shall, on the other hand, make a visual inspection of the site associated with the provision of services in the performance of the contract:

1) on 14.11.2023 at 10:00 a.m. - for the location of Arkońska 4. The beginning of the meeting in the Technical Department of the Ordering Party, in the building "W" (entrance from Broniewskiego Street),

2) on 14.11.2023 at 12:00 p.m. - for the location of A. Sokolowski 11. The beginning of the meeting in the Technical Department of the Ordering Party, staircase 11E, room BMS at 11 Sokołowskiego Street (Szczecin-Zdunowo).

1. A detailed description of the subject matter of the contract for task no. 1 is attached as Appendix no. 1 to the Terms of Reference within the scope of task no. 1. Task 1 includes:

1) compulsory liability insurance of a medicinal entity,

2) liability insurance for business and property owned.

2. The Contracting Authority provides for the possibility of awarding the contracts referred to in Article 214 (1) (7) of Pzp within 3 years from the date of award of the basic contract.

3. The orders referred to in point 2 above will consist of the repetition of services consistent with the services being the subject of this contract and services similar to them.

4. The material scope of services being the subject of the contracts referred to in point 3 above will not exceed the net value of PLN 299,079.00.

5. The contracts referred to in point 2 above shall be awarded after a separate public contract award procedure in the mode of a free hand contract.

6. The contracts referred to in point 2 above shall be awarded if there is a need to extend the insurance period due to reasonable circumstances, and in particular due to the protracted tendering procedure preventing the award within a timely manner unlimited invitation to tender either in the basic mode of the contract for the subsequent period or due to difficulties in preparing proceedings in relation to events which the Contracting Authority is not affected, in particular by force majeure (e.g. epidemic/pandemic).

7. The place of execution of the orders referred to in point 2 above will be the area of the Republic of Poland.

1. A detailed description of the subject matter of the contract for task no. 2 is attached as Appendix no. 1 to the Terms of Reference within the scope of task no. 2. Task 2 includes:

1) property insurance against all risks,

2) insurance of electronic equipment against all risks,

3) insurance of machines and equipment against failure,

4) motor insurance: OC, NNW, AC, ASS.

2. The Contracting Authority provides for the possibility of awarding the contracts referred to in Article 214 (1) (7) of Pzp within 3 years from the date of award of the basic contract.

3. The orders referred to in point 2 above will consist of the repetition of services consistent with the services being the subject of this contract and services similar to them.

4. The material scope of the services being the subject of the contracts referred to in point 3 above will not exceed the net value of PLN 186,972.00.

5. The contracts referred to in point 2 above shall be awarded after a separate public contract award procedure in the mode of a free hand contract.

6. The contracts referred to in point 2 above shall be awarded if there is a need to extend the insurance period due to reasonable circumstances, and in particular due to the protracted tendering procedure preventing the award within a timely manner unlimited invitation to tender either in the basic mode of the contract for the subsequent period or due to difficulties in preparing proceedings in relation to events which the Contracting Authority is not affected, in particular by force majeure (e.g. epidemic/pandemic).

7. The place of execution of the orders referred to in point 2 above will be the area of the Republic of Poland.
