The subject of the contract is the delivery under the name: Successive, cashless purchase of liquid fuels in ambulance and company vehicle service stations in 2022/2023, consisting in the purchase of diesel fuel and unleaded gasoline. 5. The issuance of fuels will take place in the non-cash sales system in the form of non-cash fuel (fleet) cards:

a) the cards must be issued with the registration number of cars that are

in the possession of the Ordering Party;

(b) the cards must be equipped with a magnetic strip or microprocessor;

(c) the first cards shall be issued free of charge;

d) the Contractor shall be obliged, at its own expense, to deliver to the Ordering Party's registered office the above-mentioned cards for vehicles in the Possession of the Ordering Party;

e) the data necessary to issue the cards will be provided to the Contractor after signing the contract;

f) in the event of an increase in the number of cars in the possession of the Ordering Party, the Contractor shall be obliged to deliver within 10 working days from the date of receipt of the written notification of the contract

Successive non-cash purchase of liquid fuel - diesel fuel in the town of Wyszogród or the vicinity of Wyszogród to an ambulance, permissible location of the petrol station within a radius of up to 12.0 km from the headquarters of the Wyszogród Substation, 29A Płocka Street. The Contractor must ensure that it is possible to refuel fuel from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at a minimum of 1 petrol station.

The expected purchase volume of diesel fuel – 15,000 liters.

Successive non-cash purchase of liquid fuel - diesel fuel in Gąbin for an ambulance, permissible location of the petrol station within a radius of up to 3.0 km from the headquarters of the Gąbin Substation, 19A Płocka Street. The Contractor must ensure the possibility of refueling from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at least 1 petrol station.

The expected purchase volume of diesel fuel – 12,000 liters.

Successive non-cash purchase of liquid fuel - diesel fuel in Gostynin for ambulances, permissible location of the petrol station within a radius of up to 3.0 km from the headquarters of the Gostynin Branch, 1 Przemysłowa Street. The Contractor must ensure that it is possible to refuel fuel from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at least 1 petrol station.

The expected purchase volume of diesel fuel – 16,000 liters.

Successive non-cash purchase of liquid fuel - diesel fuel in Sochaczew for ambulances, permissible location of the petrol station within a radius of up to 3.0 km from the headquarters of the Sochaczew Substation, 15-Sierpnia 6 Street. The Contractor must ensure that it is possible to refuel fuel from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at a minimum of 1 petrol station.

The expected purchase volume of diesel fuel – 20,000 liters.

Successive non-cash purchase of liquid fuel - diesel fuel in Iłów or its vicinity to an ambulance, permissible location of the petrol station within a radius of up to 12.0 km from the place of waiting of the team in Iłów, Wyzwolenia 5a Street. The Contractor must ensure that it is possible to refuel fuel from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at a minimum of 1 petrol station.

The expected purchase volume of diesel fuel – 10,000 liters.

Successive non-cash purchase of liquid fuel - diesel fuel in Sierpc to ambulances, permissible location of the petrol station within a radius of up to 3.0 km from the headquarters of the Sierpc Branch, Braci Tułodzieckich 19 Street, the Contractor must ensure the possibility of refueling from the Contractor's distributor 24 hours a day, 7 days a week (including Sundays and holidays) at least in 1 petrol station.

The expected purchase volume of diesel fuel – 26,000 liters.

Successive non-cash purchase of liquid fuels: diesel fuel and unleaded gasoline in the city of Płock for ambulances and company vehicles, permissible location of the petrol station within a radius of up to 10.0 km from the headquarters of the Płock Branch located at

Strzelecka 3. The contractor must ensure the possibility of refueling fuel from the distributor 24 hours a day, 7 days a week (including Sundays and holidays) at least 2 service stations located in the city of Płock The expected amount of diesel fuel purchase - 62,000 liters, unleaded gasoline - 1200 l.
