Description:
Electricity supply

1. The subject of the contract is the supply of electricity, as defined in the Act of 10.4.1997 - Energy Law (i.e. Journal of Laws of 2019, item 755, as amended), to 4 entities holding PPE (tapping points electricity), which are included in the following list:

- buildings of the Medical University of Bialystok in tariffs: C11– 3 PPE, C12a - 1 PPE, C23 - 6 PPE and G12w - 3 PPE,

- buildings of the University Clinical Hospital in Bialystok, in the tariff group: B23 - 2 PPE; C22b - 2 PPE,

- buildings of the University Children's Clinical Hospital in Bialystok in tariff group B23 - 2 PPE,

- buildings of the Independent Public Health Care Institution MSWiA in tariff group B23 - 2 PPE.

The list of electricity consumption points in individual tariff groups is included in Annex 2 to the ToR.

Electricity should meet the requirements, standards and technical parameters in accordance with the provisions of the Act of 10.4.1997 Energy Law and the acts enact

I. The subject of the contract is the supply of electricity, as defined in the Act of 10.4.1997 - Energy Law (i.e. Journal of Laws of 2019, item 755, as amended), to 4 entities possessing PPE (tapping points electricity), which are included in the following list:

- buildings of the Medical University of Bialystok in tariffs: C11– 3 PPE, C12a - 1 PPE, C23 - 6 PPE and G12w - 3 PPE,

- buildings of the University Clinical Hospital in Bialystok, in the tariff group: B23 - 2 PPE; C22b - 2 PPE,

- buildings of the University Children's Clinical Hospital in Bialystok in tariff group B23 - 2 PPE,

- buildings of the Independent Public Health Care Institution MSWiA in tariff group B23 - 2 PPE.

The list of electricity consumption points in individual tariff groups is included in Annex 2 to the ToR.

Electricity should meet the requirements, standards and technical parameters in accordance with the provisions of the Energy Law of 10.4.1997 as well as executive acts to this Act and Polish standards.

Annex 3 to the ToR presents the consumption of electricity in individual facilities of partners 1, 2, 3 and 4.

A detailed description of the subject of the contract, the obligations of the contractor and the contracting authority are included in Annex 6 to the ToR - contract template (general contract together with the energy sale contract).

II. Contractors who:

- are not subject to exclusion in the absence of grounds for exclusion due to non-compliance with the conditions referred to in Article. 24 paragraph 1 points 12-23 of the Act. The contracting authority does not provide for the possibility of excluding the contractor pursuant to art. 24 paragraph 5 of the Act.

III. Utilization of third party potential.

1. The contractor, which relies on the capacities of other entities, must prove to the contracting authority that, when carrying out the contract, it will have the necessary resources of these entities, in particular by presenting the undertaking of those entities to make available to it the necessary resources for the purposes of the contract. This information should be entered in the Single European Procurement Document (EAT - Annex 4 to the ToR).

2. The contracting authority shall require the contractor to indicate the part of the contract which it intends to entrust to subcontractors and the contractor to provide subcontractors. This information should be entered in the Single European Procurement Document (EAT - Annex 4 to the ToR).

3. If the change or resignation from the subcontractor concerns the entity on whose resources the contractor relied, on the principles set out in art. 22a paragraph 1

In order to demonstrate compliance with the conditions for participation in the procedure, the contractor is required to demonstrate to the Employer that the proposed other subcontractor or contractor independently fulfills them to a degree not less than the subcontractor, whose resources the contractor relied on during the contract award procedure.

IV. Make an offer

1. The contractor, according to the pattern specified by the contracting authority, prepares the offer that he submits in the public procurement procedure. The Terms of Reference with attachments are posted on the contracting authority's website www.umb.edu.pl

2. The contractor submits an offer / application for admission to the procedure, hereinafter the "application" via the form for submitting, changing, withdrawing the offer or application available on ePUAP and also available on the miniPortal. The public key necessary to encrypt the offer by the Contractor is available to contractors on the miniPortal. In the offer / application form, the contractor is required to provide the ePUAP mailbox address where correspondence will be carried out

3. The offer / application should / should be made / drawn up in Polish, in electronic form in the data format: pdf (recommended) and signed by a qualified electronic signature. The method of submitting an offer / application, including encrypting the offer, is described in the regulations of using miniPortal. The offer / application should be submitted in the original.
