1. The subject of the contract is the purchase and delivery of reagents for performing critical parameter tests (RKZ) together with the lease of analyzers for medical entities of the Podkarpackie Voivodeship listed in Chapter I of the SWZ.

2. The description of the subject of the contract is attached as Appendix No. 4 to the SWZ Detailed description of the subject of the contract (SOPZ) and Appendix No. 3 to the SWZ. Draft provisions of the contract.

The implementation of the subject of the order will take place to the Ordering Parties indicated in Annex 4.SOPZ to SWZ.

3. The Contracting Authority informs that before initiating the procedure, it conducted preliminary market consultations in accordance with Article 84 of the PPL Act. Materials concerning the preliminary market consultations carried out constitute Annex 9 to the SWZ.
