1. The subject of the contract is the provision of the service of collection, transport and utilization (neutralization) of medical waste from the activities of medical services and related research, generated by
by the Dr. Karol Jonscher Municipal Medical Center in Łódź (in the following locations: 14 Milionowa Street, 93-113 Łódź; 7/9 Przyrodnicza Street, 91-480 Łódź; 6 Leczowa Street, 93-173 Łódź; 59/58 Zarzewska Street, 93-129 Łódź
and Revolution Street 1905r. 54, 90-222 Łódź). The estimated amount of waste for a period of 12 months is set out in Annex 2 to SWZ.
The subject of the contract is the provision of the service of collection, transport and utilization (neutralization) of medical waste with the following codes:
1) 18 01 — surgical and surgical instruments and their residues (excl. 18 01 03);
2) 18 01 02 — body parts and organs and containers for blood and preservatives for its storage (excl. 18 01 03);
3) 18 01 03 — other waste which contains live pathogenic microbes or toxins thereof and other forms capable of transferring genetic material known or in which there are plausible grounds to believe that they cause diseases in humans and animals (e.g. infected diapers, naps, sleepers), excluding 18 01 80 and 18 01 82;
4) 18 01 04 — other waste than those listed in 18 01 03 (e.g. material or plaster dressings, bedding, disposable clothes, diapers);
5. 18 01 09 — medicines other than those listed in 18 01 08;
6) 15 01 10 - packs containing or contaminated residues of hazardous substances,
from the activities of medical services and related research produced by the Municipal Medical Center of Dr. Karol Jonscher in Łódź (in locations: Milionowa 14 Street, 93-113 Łódź; Naturnicza Street 7/9, 91-480 Łódź; Lecznicza Street 6, 93-173 Łódź; Zarzewska Street 59/58, 93-123-123 Łódź 9 Boat
and Revolution Street 1905r. 54, 90-222 Łódź). The estimated amount of waste for a period of 12 months is set out in Annex 2 to SWZ.
2. The scope of the service subject to this post shall include, in particular:
1) loading of waste from the place of storage at the premises of the Contracting Authority;
2) transport of waste by a special vehicle complying with the requirements for the transport of hazardous materials;
3) disposal (neutralization) of hazardous waste at a place indicated by the Contractor, in compliance with the requirements of the Waste Act of 14 December 2012 (i.e. Journal of Laws of 2023, item 1587, as amended), hereinafter referred to as the "Waste Act" and the Act of 27 April 2001.
