1. The subject of the order are successive supplies of consumables to the Hemodynamics Laboratory to the Grochowski Hospital. dr. med. Rafał Masztaka Sp. z o. o

2. A detailed description of the subject of the contract is included:

a) in Annex 2.1-2.5 to the ToR, assortment and price form;

b) in Annex 2a to the ToR - required parameters.

3. The subject of the order must comply with PN and EU standards and be admitted to trading and use in accordance with the Act of 20 May 2010 on medical devices.

4. The Employer requires that the range be offered in the form of full packaging and the delivery of intact packaging.

5. Parts of the order specified by the Employer may not be divided by Contractors. Offers not containing the full scope of the subject of the contract specified in the given part of the contract will be rejected.

6. The quantities of the assortment indicated in Annexes 2.1 to 2.5 to the ToR are for determining the value of offers and comparing them.
