1. The subject of the contract is the supply of medical equipment for "Poddębice Centrum Zdrowia" Sp. z o.o. in Poddębice, divided into 3 packages in quantities and assortment specified in Annex 2 to the SWZ – Price form and technical parameters of medical equipment.

2. The subject of the order includes:

2.1 Package No. 1 – cardiac monitor – 4 pcs., cardiac monitor trolley – 4 pcs.;

2.2 Package No. 2 – ECG recorder – 1 pc, software – 1 pc.;

2.3 Package no. 3 – ABPM recorder – 1 pc, software – 1 pc.;

3. A detailed description of the subject of the contract together with the technical parameters required by the ordering party has been specified in the Price Form and technical parameters of medical equipment – Annex 2 to the SWZ.
