1. The subject of the contract is the purchase of consumables for the CPK system and post-warranty service for the Office of Technical Inspection.

2. A detailed description of the subject of the contract has been included in the annexes to the SWZ - Price Form and Draft Agreement Provisions (contract template).

1.The scope of the subject of the contract includes:

1.1. Delivery of consumables for Fargo HDP5000, HDP8500 and MATICA MS20 mailer, in the quantities specified in the SWZ.

1.2. Maintenance service of the Card Personalization Centre (CPK) System, which includes:

- Fargo HDP5000 printer – 4 pcs.

- Fargo HDP8500 printer – 7 pcs.

- Mailer MATICA MS20 – 1 st.

- Kyocera FS-9530DN laser printer – 1 pc

- DELL OPTIPLEX 330 workstation – Windows XP Professional, Cardfive Version 9.0 Professional software – 1 pc

- DELL Vostro 320 workstation – Windows XP Professional, Maticard Pro 7000 software – 1 pc

1.3. Service of replacement of spare parts with their delivery,

1.4. Service of destruction and disposal of used consumables, i.e. YMCK tape, film, holographic tape.
