1.The subject of the contract is the delivery of envelopes intended for machine enveloping for the Centre for Digitization of Print Service in Racibórz (hereinafter referred to as CCOD) operating within the Court of Appeal in Katowice, whose task is to handle the correspondence of common courts, also referred to as "Units". The list of all Units for which the proceedings are conducted constitutes Annex 8 to the SWZ.

2.Deliveries will be made in the form of partial deliveries, based on orders placed by the Court of Appeal in Katowice, via the Ordering Party's IT system, to the e-mail address indicated by the Contractor. Detailed rules for placing orders and partial deliveries are set out in the Model Agreement constituting Annex 5 to the SWZ.

3. Partial orders will each time concern full packages (packages).

Deliveries shall be made in the form of partial deliveries, on the basis of orders placed by the Court of Appeal in Katowice, via the Ordering Party's IT system, to the e-mail address indicated by the Contractor. 4. A detailed description of the subject of the contract, including the size and scope of the contract, is attached as Annex 2 to the SWZ – OPZ - Price Form. The model of the envelopes being the subject of the contract constitutes Annex 9 to the SWZ.

5. The Ordering Party reserves the right to change the print on the envelope or resign from printing, about which the Contractor will be informed no later than 30 days before placing the order.

6. The Purchaser requires that the shelf life of the offered products be not less than 12 months from the date of a given partial delivery.

7. The envelopes which are the subject of the contract shall be intended for machine enveloping.

8. The Order shall be executed for a period of 12 months starting from the date indicated in the declaration submitted by the Ordering Party, falling no later than 3 months from the date of conclusion of the Agreement, subject to § 12 section 2 of the Agreement.

9. On the basis of the Ordering Party's statement(s), the deadline specified in paragraph 8 may be extended by the time indicated by the Ordering Party, but for a period not longer than 4 months, in the event of failure to exhaust the funds allocated for the implementation of the concluded Agreement, indicated in the provisions of § 7 section 1 letter a) or b) of the Model Agreement or failure to perform the guaranteed 50% of the anticipated order, referred to in § 2 section 5 of the Model Agreement.

10. Special cases and conditions for changing the date of performance of the contract are specified in the Model Agreement – Annex 5 to the SWZ.

11. The deadline for individual partial deliveries constitutes a non-price criterion for the evaluation of offers and will depend on the Contractor's declaration contained in the Offer Form, constituting Annex 1 to the SWZ, but may not exceed 10 business days from the date of placing the order (on the terms set out in the Model Agreement – Annex 5 to the SWZ).
