1. The scope of the service includes:

a) basic contract – Preparation of design documentation – construction project

and executive along with obtaining a decision on the permit for the implementation of a road investment for the task entitled "Extension of the provincial road No. 785 in Maluszyn – expansion of the drainage system"

b) option – performing author's supervision over the implementation of the task entitled: "Extension of the voivodship road No. 785 – expansion of the drainage system"

2. The contracting authority provides for awarding, within 3 years from the date of award of the basic contract, to the existing contractor of services or works, a contract consisting in the repetition of similar services or construction works, consistent with the subject of the basic contract, up to the amount of: - PLN 40,000.00 net.

The scope of project documentation includes the extension of the provincial road No. 785 in Maluszyn – expansion of the drainage system. The purpose of the planned extension is to improve the functioning of the existing drainage system draining rainwater and meltwater from the provincial road to the system of drainage boxes built along the road about 17+900 km of the provincial road (in the vicinity of plots 265 and 266 within Maluszyn).

STWiORB - for each of the industries based on STWiORB ZDW.

A detailed bill of quantities for each industry,

Bidding form in accordance with STWiORB,

Investment estimate,

Traffic organization design for the duration of the works,

Permanent traffic organization project,

Water law report for water services and construction of water devices along with obtaining a water law permit,

Materials for obtaining an opinion on the application for a decision on the permission to implement a road investment,

Materials for the application for a decision on the permission for the implementation of the road investment,

Information on safety and health protection,

Obtaining all other required arrangements and decisions, provisions in accordance with the detailed requirements, including the relevant protocol from the coordination meeting

Preparation of documentation and an application for a permit to derogate from the technical conditions, if the project will contain solutions that do not comply with these provisions.
