Reliance in management of the integrated home care service in favor of non-self-sufficient persons residing in the Social District B.

The service, included among the interventions to be activated in the Social District “B” of the Province of Frosinone, is carried out at the district level through the following interventions:

a) integrated health and social home care for non-self-sufficient elderly people, in order to avoid hospitalization in residential facilities;

b) relief to the family, to support family members who care for the non-self-sufficient person or to replace them in the same care responsibilities during working hours and also during periods of temporary impossibility of caring for the person not self-sufficient;

c) protected hospital resignation for persons temporarily not self-sufficient and unable to organise autonomously return to their home and the continuation of treatment, by means of organization of integrated, health and social home care interventions, programmed on the basis of an overall assessment of the needs of these subjects;

d) integrated home care, health and social care, for non-self-sufficient subjects with chronic degenerative diseases;

e) support for disabled persons who are not self-sufficient and to the family, through forms of home care and personal assistance, also lasting 24 hours and also on holidays and pre-holidays.
