The subject of the contract is: successive delivery of reagents for diagnostics carried out in the Laboratory of Cytogenetics at the Department of Hemato-Oncology Diagnostics DCOPiH divided into 3 tasks over a period of 36 months. The Contractor is excluded from the procedure pursuant to Article 109(1)(1), (4) of the Public Procurement Law and Article 108(1).

In order to confirm that the offered subject of the contract meets the requirements specified by the Contracting Authority, the Contractor submits the said means of evidence (listed in Chapter 6A of the Terms of Reference) together with the offer. If the contractor has not submitted the evidence in question or the evidence submitted is incomplete, the contracting authority shall call for its submission or supplementation within the prescribed period.
