1. The subject of the contract is the provision of health services - rental of medical and rehabilitation equipment with support.
2. A detailed description of the subject of the contract containing information about the required technical and operational parameters of the equipment and the scope of duties is specified in Annexes 2 to the ToR constituting its integral part,
3. The Employer in Annex 2 of the ToR requires the Contractor to provide the product name, manufacturer, model, year of manufacture of the subject of the contract offered by the Contractor.
4. The Contractor is obliged to fulfill the contract on the terms and conditions described in the contract template constituting Annex 6 to the ToR.
5. The Awarding Entity allows the submission of partial bids.
6. The subject of the contract was divided into 2 lots, ie:
Lot 1: gait rehabilitation and re-education system - renting with staff,
Lot 2: gait re-education equipment - rental with staff.

2. A detailed description of the subject of the contract containing information on the required technical and operational parameters of the equipment and the scope of duties is specified in Annexes 2 to the ToR constituting its integral part.
3. The Employer in Annex 2 of the ToR requires the Contractor to provide the product name, manufacturer, model, year of manufacture of the subject of the contract offered by the Contractor.
4. The Contractor is obliged to fulfill the contract on the terms and conditions described in the contract template constituting Annex 6 to the ToR.
5. Trademarks, proper names, etc. indicated in the documents - are only a quality, functional, technical and technological model regarding the subject of the order. In all cases where due to the specification of the subject of the order the origin, names of materials, devices, software, systems or their origin have been indicated, the use of materials, devices, software, systems and equivalent solutions, i.e. all materials, devices named or the software, systems used in the documentation provided by the Employer or their origin, are only used to define the standard and can be replaced by other ones with not worse technical, functional, quality, functional and aesthetic values, taking into account proper cooperation with other materials, devices and programs. Possible indicated names of products and their producers are not intended to violate the principle of fair competition and equal treatment of contractors. The concept of equivalence also applies when the Employer described the subject of the contract using standards, European technical assessments. The names used in the documentation, which indicate or could be associated with the manufacturer or company, are not intended to give preference to the solutions of a given manufacturer but to indicate a solution that should have technical and technological features not worse than those given in the technical documentation. The contracting authority in the case of offers containing equivalent solutions will verify them in terms of meeting the requirements of individual items of the technical requirements contained in Annex 2 to the ToR. The Contractor, who refers to equivalent solutions described by the Employer, is obliged to demonstrate that the supplies offered by him meet the requirements specified by the Employer. The contractor is obliged to prove in the offer the equivalence of the offered devices, software or systems. The burden of proof of equivalence is the Contractor's responsibility. The Purchaser will not recognize equivalent solutions if they are worse than the minimum quality, functional, technical and technological requirements than those specified in Annex 2 to the ToR.
6. The subject of the order is also described in Annex 2 to the ToR and in the model contract constituting Annex 6 to the ToR.

The order is carried out under Project No. RPWP.07.02.02-30-0001 / 18-00, titled "Dissemination of technologically assisted functional diagnostics and rehabilitation of children and young adults with MPD in the Greater Poland Voivodeship" as part of the Wielkopolska Regional Operational Program for 2014-2020 co-financed from the European Social Fund.
