The subject of the contract is the supply, as part of separate tasks (packages) of disposable and reusable specialized equipment used for surgical operations for the needs of BCO-SM in the assortment and quantities specified in detail in Annex 5 to the SWZ. By delivery, the Ordering Party shall mean cyclical deliveries of disposable and reusable equipment used for surgical operations during the term of the contract, in accordance with the Ordering Party's orders tailored to its current needs, at least twice a month, on dates agreed each time with the Contractor, on working days between 8:00 and 12:00. The Contractor is obliged to deliver the subject of the contract to the warehouse of the Hospital Pharmacy located at 18 Wyzwolenia Street, or the warehouse of the Hospital Pharmacy located at 21 Wyspiańskiego Street, in accordance with the address indicated in the order. In the case of assortment from a permanent base, delivery directly to the Operating Block.

The order includes, m.in. disposable neutral electrodes, cables for disposable neutural electrodes, reusable knife electrodes, disposable and reusable needle electrodes, reusable loop electrodes, disposable and reusable ball electrodes, with instrumentation, probes, bipolar tweezers, argon applicators;

The order includes, among others, neutral electrode, handle, cable for monopolar instruments, tweezers;

The order includes m.in. laparoscopic optics, suction and rinsing handle, disectors, laparoscopic scissors, trocars, reusable valves, trocar tips, reusable silicone gaskets for trocars;

The order includes, among others, removable self-expanding dentures for the esophagus, removable bariatric prostheses, self-expanding prostheses for the bile ducts, self-expanding enteric/duodenal prostheses;

The order includes m.in. circular staplers, linear with loads, staple remover, hernia mesh fixation kit;

The order includes m.in biopsy forceps, loops, injection needles, kit cleaning brushes, three-channel balloons, self-expanding pancreas and stomach stents;

The order includes m.in retractors, optical trocars, laparoscopic bags, endoscopic staplers, silicone ribbons;
