The subject of the investment is "Construction and extension of the sanitary sewage system in the Przeciszów Commune – stage VI, part 1, in Piotrowice"

The investment has been divided into 3 parts and includes:

I. Part No. 1 – "Construction of gravity sanitary sewage system with discharge collectors and equipment - sewage pumping stations." A detailed description of the subject of the contract is specified in the annexes to the SWZ, i.e. bills of quantities, STWiORB, design documentation.

II. Part No. 2 - "Purchase and delivery of mechanical equipment (WUKO) for maintenance and proper operation of the sanitary sewage system in the Municipality." The detailed scope of the subject of the Order for lot no. 2 is presented in Annex 9 to the SWZ.

III. Part No. 3 – "Performing the function of an investor's supervision inspector over construction works required by the construction law". The contract includes the provision of Investor's Supervision services over construction works carried out under task No. 1 to these proceedings.

The contracting authority has design documentation entitled: "Sanitary sewage system for the Przeciszów Commune with connections to buildings – stage IV, V, VI covering the area of part of Przeciszów and the whole of Piotrowice – Sanitary sewage system Piotrowice - stage VI". The subject of the investment is the part of the above-mentioned project – marked on the graphic attachment No. The detailed scope is determined by individual land development projects (graphic part of the project documentation), and technical / technological solutions are specified in the descriptions and detailed drawings, constituting the above-mentioned Design and bill of quantities.

The scope of work concerning Lot 1 includes:

o construction of a gravitational sanitary sewage system with pressure collectors and equipment – sewage pumping stations, with a total length of 4.5 km,

Note: gravity collectors should be built from PVC pipes SN8,

for the reconstruction of roads (substructure and asphalt surfaces) after assembly and earth works, in the area of the investment in the construction of sanitary sewage system,

Sanitary sewage pumping stations: P1 , P2

Sewage pumping stations in the amount of 2 pcs. should be equipped with pumping station monitoring devices in accordance with the bill of quantities for the sixth stage of the sanitary sewage system of the Przeciszów Commune.

1. The Contracting Authority allows the possibility of offering equipment and devices with equivalent or higher technical parameters than those listed in the SWZ. In such a case, pursuant to Article 101(4) of the Act, the Contractor who refers to solutions equivalent to those described by the Contracting Authority is obliged to demonstrate that the equipment and devices offered by it meet the requirements specified by the Contracting Authority. The basic form of demonstrating that the equipment is equivalent or has higher parameters is to provide a detailed specification.

2. As part of the offer price, the Contractor will be obliged to deliver the vehicle to the User's registered office – Gminny Zakład Wodociągów i Kanalizacji i Oczyszczania in Przeciszów, Długa 166, 32-641 Przeciszów, and to conduct training of the User's employees in the field of ongoing service of the subject of the contract. Theoretical and practical training will be carried out at the headquarters of the above-mentioned User, using the vehicle being the subject of delivery, multimedia and traditional materials. The training will concern the operation of the vehicle and the performance of current operational work. The exact date of the training will be determined by the Contracting Authority and the Contractor after signing the contract. The training will be conducted only on working days during the working hours of the User's employees, i.e. 7.00-15.00. The training will be held in Polish. The training will be confirmed in writing by the Contractor and training participants (persons designated in writing by the User) under pain of nullity.

3. The Contracting Authority requires that the offered type of vehicle is not a prototype, i.e. that it is included in the Contractor's sales offer (currently or in the past). To confirm compliance with the above requirements, the Contracting Authority requires a statement to be submitted on the offer form.

4. The subject of the contract also includes all works and obligations of the Contractor, listed in the draft provisions of the contract.

5. The parameters of the subject of the contract presented in Annex 9 to the SWZ constitute the minimum technical and quality expected by the Ordering Party. Any indication of the name of the products and their manufacturers is intended only to approximate requirements that could not be described by sufficiently precise and comprehensible terms.

6. The Ordering Party shall allow the supply of equivalent equipment with regard to the description

the subject of the contract, i.e. that the offered subject of the contract will have the same or higher technical, quality, functional and functional parameters.

7. Trademarks, proper names, etc. indicated in the description of the subject of the contract. (if

such appeared) – they are only a quality standard regarding the subject of the contract. In all cases where the names of the materials are indicated due to the specifications of the subject of the contract; It is allowed to use equivalent materials, i.e. all named materials used in the description of the subject of the contract serve only to determine the standard and may be replaced by others with no worse performance and quality parameters. In the case of tenders containing equivalent solutions, the contracting authority will verify them in terms of meeting the requirements of individual items of technical requirements contained in the annexes to the SWZ. The contractor is obliged to prove in the offer the equivalence of the offered assortment. The burden of proving equivalence is the responsibility of the Contractor. The contracting authority will not recognize equivalent solutions if they are lower than the minimum quality requirements indicated in the annexes to the SWZ.

The detailed scope of the subject of the Order for lot no. 2 is presented in Annex 9 to the SWZ.

The subject of this contract is the professional provision of consulting services and activities related to management, control and investor's supervision at the stage of construction works, the period of reporting defects and quality guarantees, over construction works carried out under a public contract relating to a selected part of the contract, and supporting the activities of the Contracting Authority.

The scope of works over which the Investor's Supervision will be performed

The order includes the provision of Investor's Supervision services over construction works carried out under task No. 1 to these proceedings entitled: "Construction of a gravitational sanitary sewage system with discharge collectors and equipment - sewage pumping stations".

The detailed scope of the subject of the contract for lot no. 3 includes construction documentation and technical specification for the execution and acceptance of works regarding part 3. 1

and the provisions of the agreement constituting Annex 5B to the SWZ.
