1. The subject of the contract is the provision of periodic technical inspections, maintenance and post-warranty repairs of the da Vinci Xi robotic surgery system, which is used by the Independent Public Health Care Facility of the Jędrzej Śniadecki Provincial Hospital in Białystok, hereinafter referred to as the Service.

2. A detailed description of the subject matter of the contract is included in Appendix No. 1 to the Terms of Reference – Detailed Offer Form.

3. The draft contract constituting Annex 11 to the SWZ is an integral part of this documentation and the provisions contained therein are treated as conditions for awarding the contract.
