NHS Rushcliffe CCG is seeking a provider for the provision of Continuing Health Care Services to the following 5 Clinical Commissioning Groups in the County Nottinghamshire; NHS Mansfield and Ashfield CCG; NHS Newark and Sherwood CCG; NHS Nottingham North and East CCG; NHS Nottingham West CCG; NHS Rushcliffe CCG.
The five CCGs serve a population of approximately 670 000 and all CCGs are linked to Nottinghamshire County Council which is an upper tier local authority, as well as the District/Borough Councils within Nottinghamshire. The named CCGs work closely with Nottingham City and Bassetlaw CCG, but they are not part of this procurement.
The CCG is keen to work with provider(s) who understand the need for that locality focus in integration with community teams and general practice, and local authority colleagues, as well as having good relationships with hospital, care home providers, home care providers and regulators. The named CCG participants are keen to explore a risk sharing agreement on the overall management of the CHC budget.
Providers will be expected to demonstrate:
How they will partner with CCGs.
How they will support CCG cost improvements as part of the QIPP programmes of each CCG, including proposals for risk sharing continuing health care spend growth.
How they will support CCG quality improvements as part of the QIPP programme in each CCG.
How the management structure they have has senior clinical leadership and oversight of the service.
How they will include service users in their service development.
How they will relate to Healthwatch.
How they can demonstrate their own internal efficiencies and value for money.
The contract will be for an initial period of 3 years, with the option to extend for a further 2 periods of 12 months. The contract start date is anticipated to be the 1.7.2015.
