1. The subject of the contract is the provision of cleaning and disinfection services for rooms and co-care services for patients in the field of medical care in the Hospital and the provision of cleaning and disinfection services in outpatient clinics and the Administration building.
2. A detailed description of the rooms and the rules for maintaining cleanliness, as well as the scope of duties of employees performing services in the Employer's facilities, are included in Annexes 1 and 2 attached to this ToR.
3. The manner of handling waste, clean and dirty underwear is included in Annexes Nos. 3 and 4 attached to this ToR.
4. The service provided must comply with the procedure - Hospital hygiene - Annex 5 attached to this ToR and procedures as listed in this Annex, which will be made available to the Contractor after signing the contract.

5. All employees (indoor, cleaners) except trainings remaining on the side of the Contractor will undergo on-the-job training conducted by authorized persons on the part of the Employer and will be familiar with the topography and applicable regulations at the hospital.
6. The Contractor is obliged to submit to the Head of the Administration Department a personal schedule of duties in the hospital, outpatient clinics and administration building not later than by the last day of each month for the following month. Changing the room or the cleaner will be possible after informing the Head of the Administration Department by phone, provided that it must be a trained person familiar with the topography and applicable regulations at the hospital.
7. All employees are required to strictly observe professional secrecy in relation to the activities performed by patients and medical activities which they may have witnessed or because of the functions they performed and the confidentiality of personal data which they could read during performance. official duties.
8. Currently, the service is performed by 33 people (the Employer does not transfer these employees).
9. In connection with the specifics of the subject of the order, the Employer pursuant to Art. 29 para. 3a. of the Public Procurement Law requires that persons performing activities of the contract indicated by the Employer (excluding cleaning the facade of the hospital building) be employed by the Contractor or Subcontractor on the basis of an employment contract throughout the duration of the contract. In the event of termination of employment of the persons referred to above before the end of the contract period, the Contractor or Subcontractor shall be obliged to promptly assign another person employed in the required form to this place.
10. At the latest on the day of signing the contract, the Contractor shall submit to the Employer a name list of persons employed by the Contractor or Subcontractor under an employment contract.
11. The Contractor shall attach to each invoice a declaration of the contractor or subcontractor on employment under the employment contract of persons performing the service for the Employer. This statement should include, in particular: the exact specification of the entity submitting the statement, the date of submitting the statement, an indication that the services provided to the Employer are performed by persons employed under an employment contract together with an indication of the number of these persons and the signature of the person authorized to make the statement on behalf of the contractor or subcontractor.
14. Payment terms - payment will be made within 30 days from the invoice date, by bank transfer to the Contractor's account indicated on the invoice.
