The subject of the contract are services in the field of voluntary group life insurance for employees and members of their families, hereinafter referred to as insurance. A detailed description of the subject of the contract is included in Annex 1 to the SWZ – Description of the subject of the contract. In this annex, the Contracting Authority specified the required insurance conditions. The Contractor may not change the required content of the insurance conditions in any way either by proposing alternative solutions or submitting a bid that does not meet all the required conditions specified by the Contracting Authority in the SWZ.
The order includes group life insurance for employees of the Specialist Hospital No. 2 in
Bytom and their family members.
The scope and conditions of insurance cover described in this SWZ and the appendices are the scope and conditions
obligatory, minimum acceptable to the Contractor.
The provisions specified in the CMS take precedence over the provisions contained in the general terms and conditions
insurance/general terms and conditions of additional insurance and are superior to the provisions of the GTC/
OWDU. In cases not specified in the GTC, the provisions of the GTC/GTC shall apply.
The contracting authority explains that a wider scope of insurance cover will not be reflected in the assessment
Deals.
4. Subject of the contract
The subject of the contract is the paid provision by the Contractor of group insurance services for
life for the employees of the Specialist Hospital No. 2 in Bytom and their family members.
5. Scope of insurance:
Employees of the Specialist Hospital No. 2 in Bytom — hereinafter referred to as "employees of the Ordering Party" or
Alternatively, "employees" and members of their families (spouses and adult children) will be able to
join 1 of the 6 variants of group life insurance, differentiated by the scope of protection and
the amount of benefits.
Obligatory scope of insurance cover, determined by the type and amount of benefits for 6 variants
benefits, hereinafter referred to as variant I, variant II, variant III, variant IV, variant V and variant VI, are set out in
Chapter VI of this Annex to the SWZ.
