The subject of the contract is the service of collection, transport with loading and unloading as well as neutralization of medical waste from subgroup 1801 (code 180102*, 180103*, 180104*, 180106*, 180108*, 180109, 180182*)" from the University Children's Clinical Hospital in Białystok, 17 J. Washington Street, hereinafter referred to as the Contracting Authority.
1. The subject of the contract is the service of collection, transport with loading and unloading as well as neutralization of medical waste from subgroup 1801 (code 180102*, 180103*, 180104*, 180106*, 180108*, 180109, 180182*)" from the L. Zamenhof University Children's Clinical Hospital in Bialystok, 17 J. Washington Street, hereinafter referred to as the Contracting Authority.

3. The expected average monthly amount of waste is approx. 8,814.79 kg, i.e. approx. 211,554.96 kg over a 24-month period. Due to the estimated quantity of the subject of the contract, it may be reduced, depending on the needs of the Ordering Party.

5. The Contractor's duties shall, in particular, be:
" Collection of waste 3 times a week (in exceptional situations 4 times a week) on Mondays, Wednesdays and Fridays from 7:00 to 9:00 or 13:30 to 15:00 and during holidays and other public holidays at least once every 72 hours.
" Receipt and disposal of medicinal products whose expiry date has expired, containing narcotic drugs, psychotropic substances and precursors of the IR group in the presence of persons responsible on the part of the Contractor.
"Weighing of waste and its loading.
" Possession of a scale with current legalization. The legalization of the scale must be valid for the entire duration of the contract.
"Collection of medical waste each time confirmed by waste transfer cards
" The Contractor will confirm the acceptance of waste using the BDO electronic system (Database
Waste)
" The records kept in the Waste Transfer Card will be the basis for issuing a VAT invoice for the performance of the service.
"Transport of waste in a manner that protects against the risk of environmental pollution, exposure to infection and in accordance with the provisions of the Act of 19 August 2011 on the transport of dangerous goods (i.e. Journal of Laws of 2020, items 154, 875)
" Responsibility before the Ordering Party and all supervisory and control authorities for the loading, transport and disposal of waste.
" In the event of a breakdown at the place indicated in the offer as the place of disposal of waste, provision of
the performance of the service in another unit, without additional charges on the part of the Contracting Authority.
" Leave the warehouse of contaminated waste after the collection of medical waste in cleanliness and proper order (residues from possible torn bags must be taken to another bag).
