The aim of this service is to provide quality care in their own home for patients who have entered the terminal phase of life, and for whom a clinical has determined they should receive care under the fast track provisions.

Fast track care will be provided so as to achieve the following overarching aims:

— To support the CCGs to enhance the quality of life for those in receipt of fast track care;

— To deliver a high-quality service ensuring service user dignity and respecting individual physical and cultural needs;

— To work in partnership in the delivery of the service with other health care professionals, including (but not exclusively);

— Local authority;

— Local acute NHS Trusts;

— Local community services providers;

— General Practitioner;

— Registered providers of care;

— To achieve the performance standards set out in this specification;

— Hospices;

— Marie Curie.
