The subject of the contract is the execution of construction works as part of the investment entitled "Modernization of road infrastructure along with lighting and water network. - can. in the City and Commune of Olkusz" on the basis of current notifications of construction works, building permits and preparation of design and cost estimate documentation and obtaining the necessary administrative decisions held by the Ordering Party. In addition, the subject of the contract is the provision of the service of a Substitute Investor with the function of the Inspector of Supervision over Parts I – XIII of the investment.

The subject of the contract is divided into 14 parts. Parts I-XIII cover construction works within a single road or road complex and are divided into tasks. Part XIV concerns the selection of the Contractor to perform the function of a Substitute Investor with the function of the Inspector of Supervision over the investment "Modernization of road infrastructure together with lighting and water and sewage network. in the city and commune of Olkusz" – PARTS I-XIII.

Part I of the subject of the contract consists of two tasks: Task 1 Reconstruction of Baczyńskiego Street in Olkusz, according to the design documentation - length of approx. 0.351 km, includes the reconstruction of a tech-class access road. D and neighboring roads with parking lots, construction of a pedestrian and bicycle route, reconstruction of sidewalks, construction of parking spaces, construction of rainwater drainage, construction of street lighting, construction of a technological channel, felling of trees and shrubs and restoration of greenery in the form of sowing biologically active areas with a grass mixture. Task 2 Reconstruction of the water supply network in the "design and build" formula - approx. 0.086 km long, includes the preparation of technical documentation necessary to submit an application for notification of works, and then the reconstruction of the water supply network along with the installation of a fire hydrant, as well as valves on the inclusions to the existing water supply. All work is carried out in accordance with PFU and OPZ. .

The subject of the contract for Part X is the construction of the road in Akacjowa Street in Żurada in the "design and build" formula - approx. 0.350 km long, including the development of design documentation along with obtaining the notification or building permit required by law, and then the reconstruction of the roadway, making roadsides from aggregate, adapting and making exits to existing buildings, ensuring water drainage from the road, development of green areas. All work is carried out in accordance with PFU and OPZ.

Part XI of the subject of the contract consists of two tasks: Task 1 - Reconstruction of the road on plot No. 684 in Olewin in the "design and build" formula - length approx. 0.215 km, includes the development of design documentation along with obtaining the notification or building permit required by law, and then the reconstruction of the roadway, making roadsides from aggregate, ensuring water drainage from the road, development of green areas. All work is carried out in accordance with PFU and OPZ. Task 2 - Reconstruction of the water supply network in Olewin in the 'design and build' formula - approx. 0.192 km long, includes the preparation of technical documentation necessary to submit an application for notification of works, reconstruction of the water supply network along the route of the existing 4th water supply along with the installation of fire hydrants and other elements. All work is carried out in accordance with PFU and OPZ.

The subject of the contract Part XII is the construction of sections of the street lighting network (power network up to 1 kV) as part of the task entitled: "Street lighting project of the streets: Jurajska, Polne Wzgórza, Spacerowa, Widokowa, Zamkowa, Na Skraju , Skalista", in accordance with the design documentation. The scope of works includes, m.in. installation of foundations, lighting poles with lighting fixtures, installation of a lighting cabinet, connection to the power grid, excavation for the cable and passages under the road and entrances, laying protective pipes, and then lighting LV cable, making connections and measurements, performing protection against shocks and overvoltage and grounding.

The subject of the Part XIII contract is the construction of a power grid up to 1 kV along with street lighting at Krucza Góra Street, in accordance with the design documentation. The scope of works includes, m.in. installation of foundations, lighting poles with lighting fixtures, installation of a lighting cabinet, connection to the power grid, excavation for the cable and passages under the road and entrances, laying protective pipes, and then lighting LV cable, making connections and measurements, performing protection against shocks and overvoltage and grounding.

The subject of the Part XIV contract is to perform the function of a Substitute Investor with the function of a Supervision Inspector for the investment "Modernization of road infrastructure with lighting and water network. - can. in the City and Commune of Olkusz - Part I - XIII. The basic duties of the Substitute Inw2estor with the function of the Supervision Inspector include, m.in.: representing the investor on the construction site by controlling the compliance of its implementation with the project or building permit, regulations and principles of technical knowledge, checking the quality of construction works and the use of products in the performance of these works in accordance with Article 10 of the Construction Law, checking and acceptance of construction works subject to cover or disappearing, participation in tests and technical acceptances of technical installations and devices as well as preparation and participation in the acceptance of finished buildings and handing them over for use, confirmation of actually performed works and removal of defects, as well as, at the investor's request, controlling construction settlements.

A detailed list of duties and scope of activities of the Substitute Investor with the function of the Supervision Inspector for the investment is specified in Annex 5.14 to the SWZ – model agreement and in Annex 2 to the agreement.

Part II of the subject of the contract consists of two tasks: Task 1 - Reconstruction of Orzeszkowa Street in Olkusz in accordance with the design documentation - middle and final section, includes reconstruction of the road roadway of Orzeszkowa Street (access road of technical class D) and sections of internal roads, reconstruction and construction of parking bays for cars and buses, reconstruction of the exit, reconstruction of pedestrian sidewalks, construction of barriers and pedestrian handrails at the sidewalks near the slopes, reconstruction of road drainage (rainwater drains and drains), reconstruction of green areas and inclusion of side roads to Orzeszkowa Street. Task 2 - Reconstruction of the water supply network in the "design and build" formula - length approx. 0.172 km, includes the preparation of technical documentation necessary to submit an application for notification of works, and then the reconstruction of the water supply network along the route of the existing water supply along with the installation of a fire hydrant, as well as valves on the inclusion of the existing water supply. All work is carried out in accordance with PFU and OPZ.

Part III of the subject of the contract consists of two tasks: Task 1 - Reconstruction of Nullo Street in Olkusz (from Słowackiego Street to Polna Street), in accordance with the design documentation - length approx. 0.290 km, includes reconstruction of the road roadway of Nullo Street, reconstruction of sidewalks, reconstruction of individual exits, reconstruction of the inclusion of a side road to Nullo Street Task 2 - Reconstruction of the sewage network with connections in the "design and build" formula - length approx. 0.198 km, It includes the preparation of technical documentation necessary to submit an application for notification of works, and then reconstruction of the sewage network along the route of the existing network along with connections to the border of plots. All work is carried out in accordance with PFU and OPZ.

Part IV of the subject of the contract consists of two tasks: Task 1 - Reconstruction of Kolberga Street in Olkusz in the "design and build" formula - length approx. 0.270 km, includes the development of design documentation along with obtaining the notification or building permit required by law, reconstruction of the roadway of Kolberga Street, reconstruction of the sidewalk, construction of parking spaces, liquidation of parking spaces at the intersection with Ściegiennego Street, reconstruction of parking spaces and bus bay (in case of technical possibilities), replacement and adjustment of wells, valve boxes and rain inlets with replacement of cast iron finials with new ones. Task 2 - Reconstruction of the water supply network in the "design and build" formula - length approx. 0.102 km, includes the preparation of technical documentation necessary to submit an application for notification of works, and then the reconstruction of the water supply network along the route of the existing water supply along with the installation of a home gate valve and inclusion in the existing water supply. All work is carried out in accordance with PFU and OPZ.

The subject of the Part V contract is the reconstruction of Dąbrowskiego Street in Olkusz in the "design and build" formula - approx. 0.162 km long. The scope of work includes in particular: development of design documentation along with obtaining the notification or building permit required by law, reconstruction of the road roadway from paving stones, execution of roadsides from paving stones, execution versus bicycle lane, adaptation of existing entrances to the property from paving stones to the road surface, replacement and regulation of wells, valve boxes and rain inlets with replacement of cast iron finials with new ones, construction of a pavement watercourse edged with an inrun curb, replacement of the existing electrical cable on the entire section of the road along with the replacement of 6 poles with LED luminaire. All work should be carried out in accordance with the PFU and OPZ.

Part VI of the subject of the contract consists of two tasks: Task 1 - Construction of sanitary sewage system in the Pomorzany housing estate: Pomorska Street, Ul. Gwarków and Stary Olkusz streets, along with the reconstruction of roads in accordance with the design documentation, includes the construction of sanitary sewage system along the following streets: Pomorska, Gwarków and Stary Olkusz in accordance with the design documentation, and then reconstruction of the bituminous road surface, reconstruction of pavements made of asphalt concrete, paving stones, prefabricated slabs, reconstruction of the surface of parking lots made of asphalt concrete and paving stones, reconstruction of green areas at the above-mentioned streets. Task 2 - Construction of a power grid with a voltage of 0.4 kV of road lighting in Gwarków Street, in accordance with the design documentation - length of approx. 0.050 km, includes the construction of a cable line supplying street lighting, construction of 1 pc. lighting pole with foundation and LED luminaire. The described tasks 2 robots are only a part of the project documentation.

Part VII of the subject of the contract consists of three tasks: Task 1 - Reconstruction of Armii Krajowej Street, in accordance with the design documentation (stage 2) - length approx. 0.246 km, includes reconstruction of the asphalt roadway together with existing exits, construction of rainwater inlets connected to the existing rainwater collector by means of drains plugged into the existing inspection wells. Task 2 - Reconstruction of armii Krajowej Street together with the "design and build" reachers, includes the development of design documentation along with obtaining the legally required notification or building permit, replacement of bituminous road surface, replacement of curbs and sidewalks, replacement of the surface of the square and turnout, construction of parking spaces, reconstruction of the pavement, regulation of wells and rain inlets. Task III - modernization of the water supply network (length approx. 0.194 km) and sanitary and rainwater sewage system in the scope of renovation of approx. 20 manholes in the "design and build" formula, includes the preparation of technical documentation necessary to submit an application for notification of works, reconstruction of the water supply network along the route of the existing water supply system, renovation of the rainwater and sanitary sewage system in the field of manholes by regulating and replacing regulatory distances together with bodies under sewer manholes. All work within the scope of Tasks 2 and 3 should be carried out in accordance with the PFU and OPZ.

Part VIII of the subject of the contract consists of three tasks: Task 1 - Reconstruction of Rabsztyńska Street in the "design and build" formula - initial section - approx. 0.700 km long, includes the development of design documentation along with obtaining the notification or building permit required by law, and then the execution of construction works, including, m.in. milling of bituminous road surface, execution of asphalt surface - wear layer, partial replacement of curbs and surfaces paving stones, regulation of wells, rain inlets and water supply valves, construction of a pedestrian and bicycle route on the section from Rabsztyńska Street to Zielona Street, construction of an intersection with Zielona Street. Task 2 - Reconstruction of the water supply network together with connections of Rabsztyńska and Sikorka streets in the "design and build" formula - length approx. 0.196 km, includes the preparation of technical documentation necessary to submit an application for notification of works, reconstruction of the water supply network along the route of the existing water supply and installation of household gate valves and on inclusions to the existing water supply. Task 3 - Reconstruction of Sikorka Street in the "design and build" formula - the middle section - approx. 0.960 km long, includes the development of design documentation along with obtaining the notification or building permit required by law, and then execution of construction works including milling of the asphalt surface, execution of asphalt surface - wear and binding layer, replacement of curbs and paving surfaces, laying paving stones in two bus bays, liquidation of the breakthrough and regulation of wells and rain inlets The whole should be done in accordance with PFU and OPZ.

Part IX of the subject of the contract consists of two tasks: Task 1 - Reconstruction of Leśna Street in the "design and build" formula - length approx. 0.490 km, includes the development of design documentation along with obtaining the notification or building permit required by law, and then the reconstruction of the roadway, the roadside made of aggregate, adaptation and execution of exits to existing buildings, ensuring water drainage from the road. All work is carried out in accordance with PFU and OPZ. Task 2 - Construction of road lighting at Leśna Street in Osiek, in accordance with the design documentation, includes the installation of foundations, lighting poles with lighting fixtures, excavation for the cable and passages under the road and entrances, laying protective pipes, and then the lighting LV cable, making connections and measurements, performing protection against shocks and overvoltage protection and grounding.
