,, IPS - implementation of electronic services for the needs of the project entitled: "Construction of IT systems supporting the implementation of the 1st Phase Intelligent Social Policy in Lodz - IPS" co-financed from the European Regional Development Fund under the Regional Operational Program of the Lodzkie Voivodship for 2014-2020 '.

The subject of the order is: granting a license, delivery, installation, configuration and implementation of software implementing electronic services along with the delivery, installation and configuration of software tools and conducting training necessary to start electronic services for residents planned under the IPS project.

1. The object of the contract is to grant a license, delivery, installation, configuration and implementation of software that provides electronic services along with the delivery, installation and configuration of software tools and conduct training necessary to start electronic services for residents planned under the IPS project.

2. The object of the contract consists of the following tasks:

2.1. Software tools:

2.1.1. Database engine, according to the requirements described in Appendix 1 to OPZ;

2.1.2. CRM software, according to the requirements described in Annex 2 to OPZ;

2.2. IPS back office software:

2.2.1. Autodiagnosis, according to the requirements described in Annex 3 to OPZ;

2.2.2. Data exchange platform, according to the requirements described in Annex 4 to OPZ;

2.2.3. MOPS applications - Information on vacancies, as per the requirements in Annex 5 to OPZ;

2.2.4. WZiSS - ŁKDR applications, according to the requirements described in Annex 6 to OPZ;

2.2.5. IPS - Analytics, according to the requirements described in Annex 7 to OPZ;

2.3. IPS front-end software

2.3.1. IPS Portal, according to the requirements described in Annex 8 to the OPZ;

2.3.2. Electronic applications, according to the requirements described in Annex 9 to the OPZ;

2.4. training

2.4.1. Training from the IPS application, according to the requirements described in Annex 10 to the OPZ;

2.4.2. Technical training, according to the requirements described in Annex 11 to OPZ.

3. As part of the implementation of configuration and installation services of the devices and software provided, the Ordering Party requires:

3.1. Preparation of the document Contract Implementation Plan;

3.2. Preparation of the Technical Design document;

3.3. Software installation and configuration;

3.4. Reconfiguration according to the needs resulting from the implementation of the order intended for the implementation of this project of the data processing environment;

3.5. Implementation of the offered solutions;

3.6. Implementation of As-built Documentation;

3.7. Submitting the solution to the Security Audit.

4. After completing the order, the Contractor will be obliged to provide warranty service and technical support for a period of not less than 12 months (in accordance with the Contractor's offer) from the date of signing the Final Acceptance Protocol by the Employer.

5. The technical and functional parameters of software and services specified in Appendices 1 to 11 should be treated as minimum requirements.

6. A detailed description of the subject of the order is included in Appendix 1 to the ToR.

7. The remaining obligations of the Contractor resulting from the performance of the contract have been specified in the model contract, constituting Annex 2 to Chapter II of the ToR.
