1) The subject of the contract is the provision of services consisting in the collection of municipal waste from inhabited properties located in the Lubicz Commune related to maintaining cleanliness and order in the Commune in 2023.

2) The service will be performed in compliance with the provisions of generally applicable law and on the terms set out in the model contract constituting Annex 4 to SWZ.

3) A detailed description of the subject of the contract is specified in Annex 5 to SWZ – Description of the subject of the contract.

1) The subject of the contract is the provision of services consisting in the collection of municipal waste from inhabited properties located in the Lubicz Commune related to maintaining cleanliness and order in the Commune in 2023. The scope of the subject of the contract includes:

1) provision of a service consisting in the collection, transport and transfer of collected municipal waste to the Municipal Installation indicated by the Contracting Authority from the owners of real estate on which residents live, located in the Lubicz Commune and accompanying services related to maintaining cleanliness and order in the area of the Lubicz Commune. The scope of the contract applies only to residential real estate located in the Lubicz Commune, in accordance with the requirements set out in this description and in the Model Agreement constituting Annex 4 to the SWZ, the service also includes the Contractor's provision of access to hard-to-reach points (in particular in winter) by organizing means of transport that enable the collection of waste from address points with a difficult location;

2) provision of a service consisting in the collection, transport and transfer of collected municipal waste to the municipal waste installation indicated by the Contracting Authority from the Selective Municipal Waste Collection Point (PSZOK);

3) provision of a service consisting in the collection, transport and transfer of collected municipal waste to the Municipal Installation indicated by the Contracting Authority of bulky waste collected directly from residents in a cycle of 2 times a year in accordance with the schedule made by the Contractor, agreed and accepted by the Contracting Entity;

4) provision of a service consisting in the collection, transport and transfer of collected municipal waste to the Municipal Installation indicated by the Ordering Party of waste from collection points for expired medicines and collection points for used batteries;

5) providing property owners with inhabited containers and bags intended for collecting individual types of waste, having colors and marking in accordance with the requirements set out in specific regulations, in this description and in the Model Agreement constituting Annex 4 to the SWZ.

6) equipping the Selective Municipal Waste Collection Point in Lubicz Górny, entry from Nowa Wieś promowa street (the area of the sewage treatment plant hereinafter referred to as PSZOK) in containers / containers for separate collection of selected waste and providing services related to maintaining them in an appropriate sanitary, orderly and technical condition, in accordance with the requirements set out in this description and in the Model Agreement constituting Annex 4 to the SWZ;

7) the provision of containers to the designated pharmacies and shops and public buildings and the provision of services related to the maintenance of them in adequate sanitary, cleanup and technical condition;

8) conducting various forms of educational activities in the field of municipal waste management;

9) carrying out checks on the implementation by the property owner of the obligation to separately collect waste, in accordance with the requirements set out in this description and in the Model Agreement constituting Annex 4 to the SWZ;

10) development and delivery of a schedule of services covered by the Subject of the contract for owners of residential real estate located in the Lubicz Commune in the event of the need to introduce changes in the dates and places of service in the schedule of municipal waste collection already distributed to property owners for 2023;

11) keeping documentation related to the activity covered by the subject of the contract.

2) The service will be performed in compliance with the applicable regulations and on the terms set out in the model contract constituting Annex 4 to the SWZ.
