The subject of the contract are construction works consisting in the implementation of the investment entitled:
Modernization of water supply and sewage systems - Stage III in the Strzelce Opolskie commune, contract 10 - including:
Part No. 1. Reconstruction and construction of the water main in Cementowa St. in Strzelce Opolskie (from SUW - MAW1 node to MWA 56 node) - contract 10.
Part No. 2. Reconstruction and construction of a water supply network in ul. Brzezińska and Ujazdowska in Strzelce Opolskie and Lipowa in the villages of Brzezina - contract 10 - and reconstruction of water supply connections on private properties.
Part No. 3. Reconstruction and construction of the sanitary sewage system in ul. Rychla in Strzelce Opolskie - contract 10.

- renovation of the measuring chambers above (K1 and K3) together with the assembly of a new fitting (installation of new flow meters) - 2 pcs
- renovation of the existing water meter well - 1 pc (at the MaW33.4 node)
- assembly of aeration and venting sets - 3 sets (chambers: K6, K7, K9)
- construction of a concrete well with an average of 1500mm with an aeration and venting set in the MaW10 node - 1 set
- restoration of green areas - approx. 469.20 m2
- restoration of bituminous surface - approx. 20.00 m2
A detailed description of the subject of the contract is included in Part III of the Terms of Reference and project documentation available at: www.swik.com.pl. Under the contract and the lump sum remuneration, the Contractor will be required to:
a) the implementation of construction works in accordance with the design documentation, technical specifications for the performance and acceptance of construction works, relevant Polish and industry standards, the Terms of Reference, principles of knowledge and construction art as well as the guidelines of the Investor's Supervision Inspectors and the Employer,
b) in the case of the subject of the contract with the participation of subcontractors, coordinating the work of subcontractors, bearing full responsibility for them,
c) ensuring technical supervision of the task being performed, supervision of staff in the field of work order and discipline,
d) proper keeping of construction documentation,
e) performance of works taking into account all conditions and orders resulting from mutual agreements and obligations,
f) organization and development of the construction base,
g) ensuring appropriate health and safety and fire protection conditions,
h) make the necessary measurements, laboratory tests, start-ups, etc., if they are necessary for putting the subject of the order into service,
i) in case of doubts as to the quality of works, equipment or materials: ensuring at its expense the performance of laboratory tests, delivery of instrumentation and ensuring human potential to check the quality and quantity of materials used to perform the contract and the quality of the works performed.
j) if, in connection with the performance of the subject of the contract, the Contractor causes damage to the Employer or other third parties, he will be obliged to repair it immediately.
k) during the implementation of the investment, the Contractor shall provide the Employer and Investor's Supervision with the opportunity to check and approve building materials that will be used to perform the subject of the contract.
l) The Contractor is obliged to restore to good condition and order the construction site, as well as (if used) roads, real estate, equipment, facilities, etc., which the Contractor violated or occupied in the performance of the subject of the contract. After completion of the works, the Contractor from the owners of the adjacent areas occupied or disturbed during the implementation of the investment, will obtain declarations on bringing these areas to a usable condition from the beginning of the implementation, which will attach to the documentation of receipt of the works.
The scope also includes the costs of:
a. Full geodetic service of completed construction works, together with preparation of a geodetic inventory of completed works and obtaining confirmation of its admission to the National Geodetic Resources,
b. Occupation of the road lane for the duration of the works,
c.

- branches of the water supply network fi 63 PE HD100 SDR11 (excavation) - approx. 1.60 mb (1 pc),
- restoration of bituminous surface - approx. 1,851.70 m2
- restoration of the pavement surface of the concrete pavement (new) - approx. 14.70 m2

b) scope of connections on the property of private owners located at ul. Brzezińska and ul. Ujazdowska in Strzelce Opolskie:
- water connections fi 32 PE HD100 SDR11 - approx. 278.69 mb (20 pcs),
- restoration of green areas - approx. 2000.23 m2
- restoring the surface of the concrete (new) cobblestones - approx. 22.84 m2
Before connecting the connection to a new network, the Contractor is to check whether the connected installation has a non-return non-return valve, and if it is not installed, install the above valve. The water connection should be made of PE 100 SDR 11 pipes with an external diameter of 32 mm, connected by welding, from the valve in front of the property owner's plot to the internal water supply system (behind the first wall of the building), i.e. to the water meter console (including the installation of a new console) with adjustable connection in the range of e.g. 130-150 mm enabling the installation of a water meter with 130mm spacing, mounted to the wall with mounting pins at least 60 mm long. Before and after the water meter (supplied by the Employer), shut-off valves should be installed in succession, and behind the valve from the side of the internal installation a non-return check valve. Water meters have a nominal diameter of DN 20 and a thread of a 1 "diameter connector. Pipe-built pipelines should be laid on 10 cm sand bed and 30 cm backfill. The connections should be placed at a depth of at least 1.40 m after the ground level. Contractor is designed to work in such a way that interruptions in the supply of water to recipients are not longer than 2 hours. In the event of the necessity of longer interruptions in the supply of water, the Employer should be notified. water after installation of the water meter by the Employer's services. After completion of construction works, the construction site should be cleaned up and restored to its original condition.

h) make the necessary measurements, laboratory tests, start-ups, etc., if they are necessary for putting the subject of the order into service,

j) if, in connection with the performance of the subject of the contract, the Contractor causes damage to the Employer or other third parties, he will be obliged to repair it immediately.
k) during the implementation of the investment, the Contractor shall provide the Employer and Investor's Supervision with the opportunity to check and approve building materials that will be used to perform the subject of the contract.
l) The Contractor is obliged to restore to good condition and order the construction site, as well as (if used) roads, real estate, equipment, facilities, etc., which the Contractor violated or occupied in the performance of the subject of the contract. After completion of the works, the Contractor from the owners of adjacent areas occupied or affected during the implementation of the investment, will obtain a declaration of bringing these areas to a usable condition from the beginning of the implementation, which will attach to the documentation of receipt of the works.
The scope of the contract also includes the costs of:
a. Full geodetic service of completed construction works, together with preparation of a geodetic inventory of completed works and obtaining confirmation of its admission to the National Geodetic Resources,
b. Occupation of the road lane for the duration of the works,
c. Road restoration in accordance with the decisions of Road Managers and detailed designs and contractual terms,
d. Traffic organization and necessary detours for the duration of the works and development of the temporary traffic organization project or any change or updating of the temporary traffic organization projects for the duration of the works,
e. Obtaining necessary decisions and reconciliations or changing out-of-date decisions and reconciliations appropriate to perform the intended construction works, which the Contractor will obtain on behalf of the Employer,
f. Preparation of as-built documentation of completed works (for each acceptance of works) containing in particular:
• design documentation with any changes,
• statement
