1. The subject of the contract are: Delivery of orthopedic implants 2. The Ordering Party divides the subject of the contract into the following parts: 2.1. task No. 1 - Hip joint endoprostheses 2.2. task No. 2 – Knee joint endoprostheses 2.3. task No. 3 – Boards and equipment for peri-prosthetic fractures 2.4. task No. 4 - Knee joint endoprosthesis rotational-hinged 2.5. Task No. 5 - Implants for joint replacements 2.6. Task No. 6 - Cementless and Cementitious Hip Endoprostheses 2.7. Task No. 7 - Cementless and Cemented Endoprostheses Knee. 2.8. task No. 8 - Primary and Revision Endoprostheses of the Shoulder Joint 2.9. Task No. 9 - Cements, systems, mixers. 3. The Contracting Authority shall not limit the number of parts for which the Contractor may submit a bid. This means that the Contractor may submit a bid for one/several/dozen/several dozen or all parts. 4. Within 5 working days from the date of signing the contract, the Contractor is obliged to set up a consignment composition of implants, supplemented each time after the operation (implantation), after submitting the wear report – applies to each task. Consignment composition of implants - in the type and quantity to be agreed with the Ordering Party. 5. The Contractor undertakes to deliver and lend for the duration of the contract a full set of instruments for implantation (applies to each task/part). The exact quantities of consignment warehouse and its content will be agreed with the Ordering Party after signing the contract. 6. A detailed description of the subject of the contract is included in: 1) Annex 1a to the SWZ, i.e. in the assortment and price form, 2) the contract template – Annex 4 to the SWZ, constituting an integral part of the SWZ.
