NHS England North (Cheshire and Merseyside) are accountable for provision of local GP services in the region and would like to commission an APMS contract to deliver essential, additional and enhanced services to practice patients.

NHS England North (Cheshire and Merseyside) is proposing to commission a APMS contract which is currently delivered from the Townfield Close, CH43 9JW a purpose built GP practice accommodation. The Provider needs to provide suitable and modern accommodation in the locality the current premises is the preferred location. This would require securing by the Provider. The commencement date for the practice is 1.10.2016.

The service is to deliver essential, additional and enhanced services to the registered patients of the practice. Core Service Payment for the registered service is 81,44 GBP per registered patient and the weighted list size is currently 6 772 patients.

Delivery of the Quality and Outcomes Framework and Directed Enhanced Services will be paid in-line with national specifications and finances. It is proposed the T and C of service, including tariff arrangements, will reflect those contained in Alternative Provider Medical Services (APMS) contract.
