A transport and support Service, 7 days a week, for patients over 60 years of age who require transport home from hospitals within the Wakefield area, in addition to low level support. This is for patients who do not meet the criteria for statutory services.

Transportation and Support Services. The Service Provider will support and enable individuals', aged 60 and over, who are assessed as medically fit to be discharged to safely return home; this could be from the Emergency Department, Ambulatory Emergency Care, Acute Assessment unit, elderly care wards, and Wakefield Intermediate Care Unit and base wards across the 3 sites of Mid Yorkshire Hospitals Trust (MYHT).

An OJEU open procurement procedure will be used to procure these services. The contract period will be for 36 months, with an option for extension of up to 24 months.
