1. The subject of the contract is the provision of care services for the elderly, lonely and disabled people as well as specialist care services for people with mental disorders at the place of residence for authorized residents of the city of Chełm.
2. Given the specificity of the subject of the contract, it is divided into the following tasks:
Task I - providing care services at the place of residence for 350 elderly, lonely and disabled people on working days and non-working days, residing in the city of Chełm.
Task II - providing specialist care services at the place of residence for 20 people with mental disorders on working days, residing in the city of Chełm.
3. In the case of Task I, the total dimension of the service is 86,206 hours. In the case of Task II, the total dimension of the service is 7 297 hours. During the performance of the contract, the specified number of hours may change for reasons beyond the control of the Employer.

3. The detailed scope of activities and rules for the provision of care services in the case of Task I is set out in Annex 1 to the ToR.
4. In the case of Task I:
1 / The contractor performs services on the basis of the Employer's written orders specifying the name, surname and address of the recipient, type and scope of services, hours, period of service and the amount of payment of the recipient for one hour of service.
2 / If it is necessary to immediately provide services to a specific person, the Contractor will start after receiving from the Employer oral, telephone information containing the name and address of the recipient, type and scope of the service, confirmed immediately by a written order.
3 / The response time to a telephone notification is max. 30 minutes.

3. The detailed scope of activities performed as part of specialist care services for people with mental disorders in the case of Task II is set out in Annex 2 to the ToR.
4. In the case of Task II:
1 / The contractor performs services on the basis of the Employer's written orders specifying the name, surname and address of the recipient, type and scope of services, hours, period of service and the amount of payment of the recipient for one hour of service.
2 / If it is necessary to immediately provide services to a specific person, the Contractor will start after receiving from the Employer oral, telephone information containing the name and address of the recipient, type and scope of the service, confirmed immediately by a written order.
