The object of the order is the purchase of medical apparatus broken down into the following parts:
PACKAGE 1 – MOBILE X-RAY MACHINE
PACKAGE 2 – ULTRASOUND MACHINE
PACKAGE 3 – ANAESTHESIA MACHINE
PACKAGE 4 – NEONATAL INTENSIVE CARE VENTILATOR
Description of the subject matter of the contract included in Appendix No. 1 to the Terms of Reference – price form with technical parameters.
The Contractor may offer only products with the CE mark that have been approved for marketing and use in accordance with the requirements of the Act of 7 April 2022 on medical devices (i.e. Journal of Laws of 2022, item 974).
The implementation of the subject matter of the contract covered by the present proceedings is covered by co-financing from the targeted subsidy of the Ministry of Health for investment purchases under the agreement concluded on 25.08.2023 No. DOI/SK/85112/6220/115/1076.
Package 1PACKAGE 1 – MOBILE X-RAY MACHINE
The Contractor's obligations with respect to the subject matter of the contract include:
1) delivery and commissioning of the subject matter of the contract,
2) carry out initial instruction and training of direct user employees in the proper operation and maintenance of the subject of the contract,
3) provision of warranty and service during the period of the quality guarantee granted.
In addition, as part of the subject matter of the contract, the Contractor will ensure full integration of the offered equipment with the functioning CLININET IT system. Integration at a level that enables:
• sending tests to and from the PACS system in the DICOM standard,
• downloading a worklist from the PACS server
• describing tests in the CGM RIS system (CLININET).
It is the responsibility of the Contractor to provide the necessary licenses to connect the camera to the CGM PACS system and to provide a warranty to ensure proper operation of the camera with the RIS/PACS system during the warranty period.

The Contracting Authority requires that the Contractor grants a warranty period of at least 24 months for the contract delivered by the Contractor as part of the subject matter of the contract.
Package 2PACKAGE 2 – ULTRASOUND MACHINE
The Contractor's obligations with respect to the subject matter of the contract include:
