1. The subject of the contract is "Improvement of the quality of health services and increase of access to medical services (construction of buildings No. B4A, B4, technical buildings along with land development) in the Provincial Specialist Hospital of Blessed Fr. J. Popiełuszko in Włocławek – Stage I, phase II of the investment". A detailed description of the subject matter of the contract is included in Appendix 2 to the Terms of Reference.

The name of the task indicated in the design documentation concerns the comprehensive reconstruction of the facilities of the Provincial Specialist Hospital in Włocławek along with land development. In the course of the investments, the original building permit was changed, the current one is attached to the Terms of Reference.

J. Popiełuszko in Włocławek – Stage I, phase II of the investment". A detailed description of the subject matter of the contract is included in Appendix 2 to the Terms of Reference.

The name of the task indicated in the design documentation concerns the comprehensive reconstruction of the facilities of the Provincial Specialist Hospital in Włocławek along with land development. In the course of the investments, the original building permit was changed, the current one is attached to the Terms of Reference.

The Ordering Party submits an exel file under the name "376-IP-00-XX-SH-X-00001-SpisZacontentGlobal" containing a detailed list of detailed design documentation with its division into phase 1 and phase 2 as part of the replacement construction and detailed documentation prepared by the design office Industria Projekt Sp. z o.o. The construction and installation works indicated in column Phase 2 constitute the scope to be performed by the Contractor. In addition, the Contractor will receive construction and executive documentation prepared by the design office Pas Projekt Sp. z o.o. for the Gatehouse building, which is also to be performed by the Contractor.

The Contracting Authority indicates that the supply of equipment described in the documentation of Medical Technology and Equipment includes the delivery and installation of:

1. Medical equipment and equipment integrally related to the structure of the building (Group 1)

2. Medical equipment and built-in equipment (Group 2).

Group 3 (Mobile medical equipment) and Group 4 (Non-medical equipment and equipment) are outside the scope of the contract.

The Contracting Authority attaches the design documentation along with the bill of quantities, which is of an auxiliary nature. Bill of quantities in the field of Medical Technology and equipment (indicated in the list of bills of quantities sections), points 3 (Mobile medical equipment) and 4 (Non-medical equipment and equipment) are outside the scope. The price of the Contractor's offer should be calculated on the basis of the design documentation.

The Contracting Authority informs that in the event of resignation from the performance of a part of the subject matter of the contract, the minimum value of the services covered by the future contract ordered by the contracting authority during the term of the contract is 30% of the total gross value specified in the contractor's offer.
