Waste treatment service classified in the classification of waste with code 19 12 12 — other wastes (including mixed substances and objects) from mechanical treatment of waste other than those mentioned in 19 12 11

The subject of the contract is the performance - under the conditions described in the contract to be concluded between the Contracting Authority and the Contractor, SWZ and in the Contractor's offer - the recovery of waste collected by the Contractor from the Contracting entity, classified under code 19 12 12 - other waste (including mixed substances and objects) from mechanical treatment of waste other than those listed in 19 12 11 divided into tasks (parts) in the types and quantities specified below. Waste may be subjected only to R1 and/or R12 recovery processes, as defined in the Waste Act of 14.12.2012. (i.e. Journal of Laws 2022. 699, as amended)/Annex IA to Regulation (EC) No 1013/2006 of the European Parliament and of the Council of 14.06.2006. on shipments of waste (OJ EU. L 190/1 as late as amended), whereby 'submission to a recovery process' means the beginning and completion of that process.

Part No. 1 - Waste in the amount of 4500 Mg (+/-) 10 Mg

The subject of the contract is the performance - under the conditions described in the contract to be concluded between the Contracting Authority and the Contractor, SWZ and in the Contractor's offer - the recovery of waste collected by the Contractor from the Contracting entity, classified under code 19 12 12 - other waste (including mixed substances and objects) from mechanical treatment of waste other than those listed in 19 12 11 divided into tasks (parts) in the types and quantities specified below. Waste may be subjected only to R1 and/or R12 recovery processes, as defined in the Waste Act of 14.12.2012. (i.e. Journal of Laws 2022. 699, as amended)/Annex IA to Regulation (EC) No 1013/2006 of the European Parliament and of the Council of 14.06.2006. on shipments of waste (OJ EU. L 190/1 as late as amended), whereby 'submission to a recovery process' means the beginning and completion of that process.

Order parts:

part no. 1 – in the amount of 4500 Mg (+/-) 10 Mg (quantitative differences may result from warehouse conditions and from the technical and technological solutions of the weighing system), but not less than 40 Mg (minimum benefit)

Waste packaging: Baled waste, i.e. pressed and then balked with steel wire.

The Contractor must provide in the tender form (Annex 1 to SWZ) the recovery process to which the waste will be subjected.

1. Collection of waste:

The transfer and receipt of Waste shall be carried out solely on the basis and in accordance with written [form under the rigour of annulment] of the Ordering Party unit orders to the Contractor and served by fax or e-mail, specifying the quantity of Waste and the time limit on which The Ordering Authority expects their receipt.

The Ordering Party will place unit orders when the amount of Waste collected by him, which is to be collected on one day, will be at least 18 Mg in the case of parts 1, 2.

The Contractor shall receive the waste while maintaining the following technical infrastructure requirements at the contracting plant:

• gross weight of a single vehicle entering (empty vehicle for loading) into the premises of the Contracting Authority shall not exceed 18 tonnes

• distances between the extreme axles of a single vehicle shall not exceed a length of 16 metres and the length of the vehicle shall not exceed 18 m

2. After weighing the vehicle loaded with Waste, the Ordering Party shall issue a release document from the warehouse.

3. The Contracting Authority shall draw up a Waste Transfer Card [KPO] for each batch of Waste collected from its plant by transport to the Contractor. These cards will be issued in the form and will contain information in accordance with the Waste Act. The Contractor shall be responsible for confirming by the Operator of the Installation in which the recovery process will be carried out, the weight and type of Waste specified in each Waste Transfer Card issued by the Ordering Party, in accordance with the Waste Act. If the shipment of Waste will be of an international nature, the documents specified in Regulation (EC) No. 1013/2006 of the European Parliament and of the Council of the European Union of 14 June 2006 on shipments of waste (Journal of Laws of the European Union) will be drawn up. L 190/1 as late Deaths). The documents will be prepared in accordance with the requirements of the above-mentioned Regulation.
