The subject of the contract is the provision of water rescue and first aid services in the Sports Facilities Complexes (ZOS) belonging to the Ursynów Sports and Recreation Centre, i.e.

1) Sports Facilities Complex "Koncertowa" at 4 Koncertowa Street,

2) Sports Facilities Complex "Aqua Relaks" at 3 Pod Strzechą Street,

3) "Hirszfelda" Sports Facilities Complex at 11 Hirszfelda Street.

1. The subject of the contract is the provision of water rescue and first aid services in the Sports Facilities Complexes (ZOS) belonging to the Ursynów Sports and Recreation Centre, i.e.

1) Sports Facilities Complex "Koncertowa" at 4 Koncertowa Street,

2) Sports Facilities Complex "Aqua Relaks" at 3 Pod Strzechą Street,

3) "Hirszfelda" Sports Facilities Complex at 11 Hirszfelda Street.

2. The expected total number of rescue hours during the contract implementation period is a maximum of 33,224.25 hours, including:

a) Concert Swimming Pool – 10,456.50;

b) Aqua Relax Swimming Pool – 14,129.50;

c) Hirszfeld Swimming Pool – 5,034.50;

d) Additional hours resulting from the necessity of substitution of full-time employees: 3,603.75 (Aqua Relaks Swimming Pool – 2,000 h, Hirszfeld Swimming Pool – 1,603.25 h).

3. The implementation of the subject matter of the contract may be entrusted only to lifeguards who have:

a) a valid lifeguard licence granted by an authorised entity in accordance with the Act of 18 August 2011 on the safety of persons staying in water areas and implementing acts to this Act;

b) completed a course in qualified first aid (KPP) enabling the performance of activities specified in Article 14(1)-(8) of the Act of 8 September 2006 on the National Emergency Medical Services;

c) a valid medical examination confirming the ability to work as a lifeguard, confirmed by a certificate from an occupational medicine clinic confirming the ability to work as a lifeguard and whose health condition allows to provide qualified first aid in accordance with Article 13 (1) (4) of the National Emergency Medical Services Act;

d) up-to-date medical examinations for sanitary and epidemiological purposes;

e) training in health and safety and fire protection regulations;

f) pass a positive verification (carried out by an authorised employee of UCSiR on the basis of Appendix 2 to the agreement) in the Register of Sexual Offenders;

g) demonstrate a sufficient knowledge of the Polish language to perform this profession, enabling communication with the client, and submit an appropriate statement in this regard.

4. Description of Objects:

1) Concert Swimming Pool:

a) a sports basin with a size of 25x12.5m, depth 1.0-1.6m (6 tracks).

2) Aqua Relax Swimming Pool:

a) a sports basin with a size of 25x16m, depth 1.2-1.8m (6 tracks),

b) recreational basin, irregular shape F=45m², depth 0.9-1.1m,

c) a slide with a braking basin,

d) Whirlpool bathtub for 5 people,

e) Roman and Finnish saunas.

3) Hirszfeld Swimming Pool:

a) a sports basin with a size of 25x12.5m, depth 1.0-1.6m (6 tracks),

b) a slide with a braking basin,

c) Whirlpool bathtub for 5 people.
