1. The subject of the contract is group insurance on the life and health of employees and members of the families of employees of the Hospital of St. Jadwiga Śląska in Trzebnica.
2. The order was divided into two parts, i.e.:
Part I:
Package I - Group life insurance for employees and members of the families of employees of St. Jadwiga Śląska Hospital in Trzebnica.
Part II:
Package II - Group health insurance of employees and members of the families of employees of St. Jadwiga Śląska Hospital in Trzebnica.
3. The Contracting Authority does not guarantee that all employees employed and members of the families of employees of the Ordering Party will enter into group life and health insurance.
4. There may be changes in the amount of insured persons during the course of the contract.
5. If the employee or family member of the employee resigns from insurance, the Ordering Party is not obliged to pay for them the insurance premium
