The subject of the contract is the service of group life insurance for employees and members of their families within 3 insurance scopes.

1. The subject of the contract is the service of group life insurance for employees and members of their families under 3 insurance scopes.

2. The Contracting Authority requires that the implementation of the subject of the contract takes place on the terms and conditions specified in the model contract and in accordance with the description of the subject of the contract.

3. Insurance is voluntary. Persons who meet the criterion of belonging to the group may join the insurance or resign from insurance at any time during the term of the contract.

4. Every employee of the Contracting Authority may join the insurance, regardless of the form of employment (employment contract, act of appointment) and regardless of age. The age and gender structure of all employees of the Ordering Party (647 persons) is included in Appendix 1 to OPZ – Structure of employees.

5. The contracting authority does not guarantee that all employees will benefit from the possibility of joining group insurance.
