Provision of patient transport services to the Dr. Ludwik Rydygier Provincial Hospital in Suwałki

The subject of the contract is the provision of transport services to the Dr. Ludwik Rydygier Provincial Hospital in Suwałki in the quantities and assortment specified in Annex 5 to the SWZ

The subject of the contract is specified in detail in the description of the subject of the contract constituting Annex 6 to this SWZ.

Part 1

The subject of the contract is the service of ambulance transport with basic unit "P" in the amount specified in Annex 5 to the SWZ and in accordance with the conditions set out in Annex 6 to the SWZ

Part 2

The subject of the contract is the medical transport service of the specialist team "S" in the amount specified in Annex 5 to the SWZ and in accordance with the conditions set out in Annex 6 to the SWZ

Part 3

The subject of the contract is the transport service of the transport unit "T" in the amount specified in Annex No. 5 to the SWZ and in accordance with the conditions set out in Annex No. 6 to the SWZ

Part 4

The subject of the contract is the service of blood and biological materials in the amount specified in Annex No. 5 to the SWZ and in accordance with the conditions set out in Annex No. 6 to the SWZ
