3.1 The subject of the contract is the provision of sanitary transport services within the meaning of Article 5 (33a) of the Act of 27 August 2004 on public funded health care services (i.e. OJ 2019 item 1373 as amended), i.e. the transport of patients (transport in a state of emergency life, for consultations and diagnostic examinations, transfer of the patient to another medicinal entity, transport home, transport of dialysis patients in a hospital dialysis station), biological materials (blood and blood materials, biological material for testing or other materials/ medical devices or medical records), all day/ 7 days a week/ 365 days a year.

Sanitary transport — with a specialist team, an estimated amount of approximately 8,000 km and 650 hours of operation,

Sanitary transport — depending on your needs with a paramedic, without medical care and transport of blood, blood materials, biological materials and others (driver only)

Estimated amount of km: 80,000; time approx. 7,000 h/year — with lifeguard,

Estimated quantity of km: 25,000; time approx. 1,500 h/year — with a paramedic,

Estimated km: 15,000; time approx. 550 h/year — transport of blood, blood materials, biological materials and others (driver only).

Transport of dialysis patients - the provision of sanitary transport services (dialysis persons in the hospital dialysis station — 44-100 Gliwice ul. Kościuszki 29) within the meaning of Article 5 point 33a) of the Act of 27 August 2004 on public funded health care services (i.e. OJ 2021 item 1285 as amended), i.e. the transport of patients in a non-life-threatening condition all day/ 7 days per week/ 365 days per year according to a fixed schedule.

Number of patients served by dialysis station: approx. 72 people

Average number of dialysis procedures performed per month: approx. 900

Projected number of kilometres during the year: 110,000 seated people + 10,000 lying
