1. The subject of the contract is the supply of medical equipment for the Department of Neurology with the Stroke Subunit of the Zofia z Zamoyska Tarnowska Provincial Hospital in Tarnobrzeg, included in 16 packages.
2. A detailed description of the subject of the contract is included in Annex 2 to the SWZ – Description of the subject of the contract and Annex 3 to the SWZ – Price form.
3. The Contractor is obliged to complete all the items in Appendix 2 and 3 to the SWZ.
4. The rules for the implementation of the contract in question are specified in the "Draft contract" constituting Annex 9 to the SWZ.
Package 11. The subject of the contract includes:
1) Purchase of medical equipment, including:
Package 1 Cardiac monitor with capnograph
2) Delivery of the above-mentioned equipment;
3) Assembly/installation, commissioning, testing and checking of the above-mentioned medical equipment;
4) Training of designated employees of the Ordering Party in the use of the above-mentioned equipment;
5) Training of technical personnel in the field of maintenance and service of the above-mentioned equipment;
6) Warranty service of the above-mentioned equipment, on the terms set out in § 8;
7) Delivery of warranty cards of the above-mentioned equipment;
8) Providing technical passports of the above-mentioned equipment;
9) Delivery of instructions for use of purchased devices and medical equipment in paper version in Polish, in electronic version in Polish and technical (service) documentation;
10) Obtaining all necessary permits for the use of medical devices and equipment as part of the remuneration specified in § 3 clause 1.
2. Medical devices constituting the subject of this agreement must meet the requirements of the current legal status, in particular those introduced by the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974).
3. The parameters of the standard indicated by the Ordering Party present the technical, operational, utility, functional conditions and other features relevant to the subject of the contract, while the indicated brand or trade name specifies the class of the product, not a specific manufacturer. The Contractor may offer equivalent products, provided that they have no worse technical parameters than those described and given in the SWZ.
4. The Contractor shall be responsible for the quality, compliance with the technical and quality conditions described for the subject of the contract.
5. The rules for the implementation of the contract in question are specified in the "Draft Agreement" constituting Annex 9 to the SWZ.
Package 21. The subject of the contract includes:
1) Purchase of medical equipment, including:
Package 2 Premium ultrasound machine with 2 probes
2) Delivery of the above-mentioned equipment;
3) Assembly/installation, commissioning, testing and checking of the above-mentioned medical equipment;
4) Training of designated employees of the Ordering Party in the use of the above-mentioned equipment;
5) Training of technical personnel in the field of maintenance and service of the above-mentioned equipment;
6) Warranty service of the above-mentioned equipment, on the terms set out in § 8;
