The subject of the order is the delivery, installation and implementation of hospital applications and systems, Local Communication Software, Authorization System and LAN components - ETAP III

A wider order performed as ETAP III under the names of:

1. Provide the authorization system (chip cards, chip readers, software, certificates).

2. Provide the configuration and commissioning of information processing support systems in medicinal entities (hospital and specialty applications and systems).

For the project “Equipment of the IT environments of provincial, county and urban medicinal entities with IT tools enabling the implementation of EDM and the creation of a network of data exchange between the curative entities of the local government”

The object of the order for Part I is:

1. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed with the Ordering Party in the Specialist Health Care Team of the Mother and Child in Poznan with the existing Regional Platform enabling the transmission to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as enabling remote registration to the services provided by the Contracting Authority through the Platform according to with a detailed description of the object of the contract set out in Annex 13 to the OPZ — item 1

2. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part X is the delivery, installation, installation and configuration of the LAN, and UPS power supply for the Stanisław Staszica Specialist Hospital, Rydygiera 1 Street; 64-920 Piła.

The object of the order for Part XI is the delivery, installation, assembly and configuration of the LAN, and the UPS for the Care and Treatment Department.

The object of the order for part XII is the delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, certified certificate for the Greater Poland Voivodeship - pc.1

The object of the order for Part II is:

1. Delivery, installation and implementation of the Local Communication Software (LOK) enabling the integration of the HIS Hospital Information System installed at the Ordering Party in the Rehabilitation and Cardiac Hospital in Kowanówek with the existing Regional Platform for enabling upload to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with the detailed description the object of the order set out in Annex 13 to the OPZ — item 1

2. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part III is:

1. Delivery, installation and implementation of HIS System modules/licenses at the Contracting Authority Self-Public Health Care Department in Szamotules to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order contained in Annex 3 to OPZ — Description of the requirements for the HIS System for Part III — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed at the Contracting Authority Self-Public Health Care Department in Szamotules with the existing Regional Platform relying on enabling the transmission to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with detailed description of the object of the order set out in Annex 13 of the OPZ — item 1

3. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part IV is:

1. Delivery, installation and implementation of HIS System modules/licenses at the Contracting Authority of Public Health Care in Inter-Revenue in order to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order contained in Annex 4 to OPZ — Description of the requirements for the HIS System for Part IV — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling the integration of the HIS Hospital Information System installed at the Contracting Authority Self-Public Health Care Department in Inter-Revenue with the existing Regional Platform relying on enabling the transmission to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with detailed description of the object of the order set out in Annex 13 of the OPZ — item 1

3. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part V is:

1. delivery, installation and implementation of the HIS System modules/licenses at the Ordering Hospital District in Jarocinie Sp. z o.o. to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order set out in Annex 5 to the OPZ — Description of the requirements for the HIS System for Part V - pcs. 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed at the Ordering District Hospital in Jarocina Sp. z o.o. with the existing Regional Platform consisting in enabling the transmission to Electronic Medical Documents (EDM) platform produced in HIS software according to the standards required by the Platform as well as allowing remote registration for services provided by the Contracting Authority through the Platform in accordance with the detailed description of the subject procurement set out in Annex 13 OPZ — item 1

3. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part VI is:

1. delivery, installation and implementation of the HIS System modules/licenses at the Ordering Party of Health Care Plant in Ostrów Wielkopolski in order to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order contained in Annex 6 to OPZ — Description of the requirements for the HIS System for Part VI — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed at the Ordering Party of Health Care Plant in Ostrów Wielkopolski with the existing Regional Platform consisting of enabling upload to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with the detailed description the object of the order set out in Annex 13 of the OPZ — item 1

The object of the order for Part VII is:

1. delivery, installation and implementation of the HIS System modules/licenses at the Ordering District Hospital named after Tadeusz Maliński Sp. z o.o. in Śrem to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order contained in Annex 7 to OPZ — Description of the requirements for the HIS System for Part VII — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed at the Ordering District Hospital named after Tadeusz Maliński Sp. z o.o. in Śrem with the existing Regional Platform relying on enabling the transmission to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with detailed description of the object of the order set out in Annex 13 of the OPZ — item 1

Delivery, installation and implementation of HIS System modules/licenses at Ordering Hospital District Sp. z o.o., in September to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the contract set out in Annex 8 to the OPZ — Description of the requirements for the HIS System for Part VIII — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling the integration of the HIS Hospital Information System installed at the Ordering Hospital District Sp. z o.o. in Września with the existing Regional Platform consisting in enabling the transmission to Electronic Medical Documents (EDM) platform produced in HIS software according to the standards required by the Platform as well as allowing remote registration for services provided by the Contracting Authority through the Platform in accordance with the detailed description of the subject procurement set out in Annex 13 OPZ — item 1

3. delivery of the Authorization System consisting of chip (microprocessor) card, chip reader (microprocessor), software, qualified certificate - pc.1

The object of the order for Part IX is:

1. Delivery, installation and implementation of modules/licenses of the HIS System at the Contracting Authority of Louis Perzyna Provincial Complex Hospital in Kalisz in order to enable the issuance of Electronic Medical Documentation and e-Registration in accordance with the detailed description of the object of the order contained in Annex 9 to OPZ — Description of the requirements for the HIS System for Part IX — item 1

2. Delivery, installation and implementation of the Local Communication Software (LOK) enabling integration of the HIS Hospital Information System installed at the Ordering Officer Louis Perzyna Provincial Complex Hospital in Kalisz with the existing Regional Platform for enabling the possibility of enabling the provision of an existing Regional Platform for enabling the possibility of enabling upload to the Platform of Electronic Medical Documents (EDM) produced in HIS software according to the standards required by the Platform as well as to enable remote registration to the services provided by the Contracting Authority through the Platform in accordance with the detailed description the object of the order set out in Annex 13 of the OPZ — item 1
