1. The subject of this contract is: Delivery of medical equipment to specialist clinics IV for the Health Care Team, Baczyńskiego 1, 55-200 Oława.

2. The scope of the contract shall cover 11 lots.

3. Quantities, characteristics and technical and functional parameters of the subject of the contract are specified in the description of the subject of the contract – Annex 2-1 – 2-16 (respectively for individual tasks) and Annexes No. 1, 5 to the specification of the terms of the contract (SWZ).

4. The subject of the contract shall comply with the requirements laid down by the Contracting Authority in:

a) Act on Medical Devices of 7.04.2022. (consolidated text, Journal of Laws of 2022, item 974, as amended),

b) Regulation of the Minister of Health of 17 February 2016 on essential requirements and procedures for conformity assessment of medical devices (Dz.U. of 2016, item 211 with later. Deaths) ;

c) it should meet the technical requirements contained in the SWZ and annexes to the SWZ,

Task No. 1 - Analyzer of nitric oxide in exhaled air in accordance with Annex No. 2-1 to the SWZ

Task No. 15 - Torque platform in accordance with Annex 2-15 to the SWZ

Task No. 16 - Set for exercise tests with defibrillator in accordance with Annex No. 2-16 to the SWZ

Task No. 3 - Walker for gait reeducation in accordance with Appendix No. 2-3 to SWZ

Task No. 4 - Rehabilitation walker type "Pulpit" in accordance with Annex No. 2-4 to the SWZ

Task No. 5 - Rehabilitation treadmill with accessories in accordance with Annex No. 2-5 to the SWZ

Task No. 6 - Dermatoscope in accordance with Appendix No. 2-6 to the SWZ

Task No. 7 - Echocardiograph in accordance with Annex No. 2-7 to the SWZ

Task No. 8 - Electromyograph in accordance with Appendix No. 2-8 to the SWZ

Task No. 11 - Ophthalmoscope in accordance with Appendix No. 2-11 to the SWZ

Task No. 12 - Dynamographic platform in accordance with Annex No. 2-12 to the SWZ
