1. The subject of the contract includes:
1.1. purchase of a tomograph;
1.2. delivery with transport insurance;
1.3. disassembly and utilization of the old apparatus;
1.4. execution of a construction and technological design for the renovation and adaptation of premises;
1.5. performance of construction works related to the adaptation of rooms intended for the tomograph installation;
1.6. installation, commissioning and commissioning of the tomograph for clinical use;
1.7. instructional training at the Employer's headquarters.
2. The technical (border and rated) requirements of the tomograph are set out in Annex 1 to the ToR.

2. The technical (border and rated) requirements of the tomograph are set out in Annex 1 to the ToR.
The contractor is obliged to complete all items of Annex 1 in accordance with the parameters offered.
In the absence of data on the boundary conditions (Annex 1), the Purchaser will consider that the offered configuration of the offered apparatus lacks the required parameter. Consequently, failure to meet the absolutely required conditions will result in rejection of the offer.
However, the lack of data in the column regarding the evaluated parameters will result in the awarding of "0 points" in this item.
The subject of the contract includes the development of a construction and technological design and execution of construction works in the field of: execution of construction and technological documentation in the construction and building, sanitary, electrical and teletechnical industries in accordance with the technical and operational documentation of the new tomograph, as well as carrying out renovation and adaptation works in accordance with the construction and construction documentation technology. The contractor is required to agree on the construction and technological design with the Employer and the Tomograph Supplier. The Contractor shall attach to the construction and technological documentation the premises inventory as well as the material and financial schedule of the execution and settlement of the works performed. The detailed scope of the subject of the order is specified in the Functional and Utility Program - constituting Annex 2 to this ToR.
It is allowed to start renovation and adaptation works during the implementation of design works in consultation with the Supervision Inspector, user and supplier of the tomograph.
4. The main subject of the contract must meet the requirements set out in the Act of 20.5.2010 on medical devices, in particular be marked with the CE mark, and if the conformity assessment was carried out under the supervision of a notified body, the appropriate identification number should be placed next to the CE mark notified body.
5. The minimum required warranty period for the main subject of the contract - 36 months from the date of signing the final acceptance report of the subject of the contract. The Employer requires that the Contractor during the warranty period perform free of charge necessary technical inspections and apparatus maintenance at the frequency recommended by the manufacturer.
The minimum warranty period for completed design and construction works - 36 months from the date of the protocol acceptance of works.
6. The term of the contract in the field of:
1) carrying out design and construction works - up to 15 weeks from the date of the contract;
2.) delivery and installation of the tomograph - up to 21 weeks from the date of the contract;
3) putting the tomograph into clinical operation - no later than within 14 days from the date of completing the tomograph installation;
4) training for medical, engineering and technical personnel on dates agreed with the Employer.
