The subject of the contract is the sale and delivery of assortment used in obstetrics in the following groups:

Group 1 – Non-woven napkins

Group 2 – Birth kits

Group 3 – Caesarean section kits

Group 4 – Cover for operating light holder

Group 5 – Surgical gowns

Group 6 – Self-adhesive organizer for fixing wires and drains

Group 1

Nonwoven drape

When selecting and evaluating the submitted offers, the Ordering Party shall be guided by the following criteria:

Method of assessment:

1. Price – criterion weight - 60%.

The criterion: price – indicator C – will be calculated according to the formula.

C = ( Cn / Cb ) x 60

Where:

Cn – offer with the lowest gross price.

2. Current delivery date – criterion weight – 40%.

The criterion of the term of current deliveries – indicator D – will be calculated according to the formula:

D = ( Dp / Dmax ) x 40

Where:

Dp – the number of points received by the examined offer.

Dmax – the maximum number of points to be obtained within the criterion.

Number of points awarded under the Dp:

1) Current delivery date - 7 working days – 0 points.

2) Current delivery time - 6 working days – 10 points.

3) Current delivery time - 5 working days - 30 points.

4) Delivery time - 4 working days – 60 pts.

5) Current delivery time – up to 3 working days – 100 points.

Under the criterion, the contractor may receive a maximum of 100 points = 40%.

3) - Evaluation of the offer – the O indicator will be calculated according to the formula:

D - term of current deliveries.

The Contracting Authority shall exclude the Contractors on the basis of:

- Article 108(1) of the Public Procurement Law,

- Article 7(1) of the Act of 13 April 2022 on special solutions for counteracting support for aggression against Ukraine and serving to protect national security,

- Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine. EU No L 229, 31.7.2014, p. 1), hereinafter: Regulation 833/2014, as amended by Council Regulation (EU) 2022/576 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine. EU No L 111, 8.4.2022, p. 1), hereinafter: Regulation 2022/576.

The Contracting Authority shall additionally exclude the Contractor pursuant to Article 109(1)(4) of the Public Procurement Law.

When selecting and evaluating the submitted offers, the Ordering Party shall be guided by the following criteria:
