Description:
The provision of cleaning services and assistance activities for the District Hospital in Rawicz Sp. z o.o.

The subject of the order is the cleaning services of buildings and the performance of patient assistance for the District Hospital in Rawicz Sp. z o.o.

For a detailed description of the item of the order, see Annex 2 of the SWZ.

1) The subject of the contract is the provision of daily, 24-hour cleaning of premises in the buildings of the County Hospital in Rawicz Sp. z o.o. and the performance of patient assistance, provided both on weekdays and festive days, for a period of 36 months from 1 day October 2021 to 30 September 2024

2) For a detailed description of the object of the order, see Annex 2 of the SWZ.

3) The service will be carried out by the Contractor at the office of the Ordering Authority — 63-900 Rawicz, 6 Grotto Rowecki Street.

4) The Contracting Authority shall require that the execution of the object of the contract be carried out under the terms and conditions set out in the template of the contract and as described in the subject matter

5. The Contracting Authority requires the execution of the order by means of disinfectants and washing agents indicated in Annex 3 to the SWZ and by means of its own equipment and equipment. The Contractor shall, in his own right and at his own expense, provide the facilities, equipment and disinfectants and washing agents necessary for the performance of the service.

6) The Contracting Authority, acting on the basis of Article 95 Pzp, requires that the Contractor, subcontractors or further subcontractors shall employ employees employed under an employment contract within the meaning of the provisions of the Labor Code, with direct execution of the subject of the contract the following activities necessary for the performance of the subject of the contract, i.e.: the cleaning activities of the premises of the Contracting Authority, the employment of persons under the employment contract should continue uninterrupted throughout the period of the implementation of this contract.

7) The Contracting Authority requires the Contractor to have an adequate amount of staff to perform the service to the extent specified by the Contracting Authority. The Contracting Authority shall require the performance of the service with the maximum number of jobs required to perform the due work schedule specified by the Contracting Authority in Annex 3 to the contract.

8) The Contractor is obliged to have liability insurance in connection with the provision of the contract service. The Contractor shall be obliged to maintain the continuity of insurance coverage throughout the duration of the contract. A copy of the policy certified for compliance with the original Contractor shall be obliged to provide the Ordering Party at the latest on the date of signing the contract.

Pursuant to Article 13 (1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals in connection with the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Regulation on data protection) (OJ EU L119, 4.5.2016, p. 1), hereinafter “GDPR” — thus personal data provided by the Contractor will be processed in accordance with the GDPR and in accordance with national regulations. The personal data of the Contractor shall be processed pursuant to Article 6 (1) (c) of the GDPR for a purpose related to the public contract award procedure No NLO-3820-01/PN/21 conducted in basic mode without negotiation. Recipients of the personal data provided by the Contractor will be persons or entities to whom documentation of the procedure in accordance with Article 8 and Article 96 (3) of the Law of Public Procurement (Journal of Laws 2019, item 2019), as well as Article 6 of the Act of September 6, 2001 on Access to public information. The Contractor's personal data contained in the Protocol of Procedure shall be kept for a period of 4 years from the date of completion of the procurement procedure If the duration of the contract exceeds 4 years, the storage period shall cover the entire duration of the contract. The information clause referred to in Article 13 (1) and (2) of the GDPR shall be found in Chapter III of the Procurement Terms Specification.
