Purchase and delivery of an accelerator under the long-term program "National Oncological Strategy entitled "Retrofitting of radiotherapy facilities - replacement of accelerators".

1. The subject of the contract is: "Purchase and delivery of an accelerator under the multiannual program "National Oncological Strategy entitled "Retrofitting of radiotherapy facilities - replacement of accelerators".

2. As part of the performance of the subject of the contract, the Contractor undertakes to:

a) delivery of medical devices/equipment constituting the subject of the contract together with the documents required in the SWZ to the Ordering Party's registered office,

b) uninstallation of the accelerator with disposal,

c) adaptation of the room intended for the installation of the device,

d) assembly of the medical device/apparatus in the place indicated by the Ordering Party,

e) installation and commissioning of the accelerator device along with software configuration

f) instruct the Ordering Party's staff in the operation and rational operation of the supplied medical equipment,

g) delivery together with the device/medical apparatus of a user manual in Polish, technical passports, data sheets, warranty cards, technical documentation necessary for the proper operation of the device/apparatus, rules for the provision of services by an authorized service during the warranty and post-warranty period, schedule of inspections, list of consumables used in the current operation of the subject of the contract, other documentation required by law, implementation other conditions specified in the SWZ together with annexes.
