The procedure is conducted in an open tender procedure pursuant to Article 132 of the PPL.The subject of the contract is the supply of medical devices for the Operating Block according to the following parts: Task 1 - Task 14. A detailed description of the subject of the contract of the above-mentioned parts has been listed and described in the "Price Form" (Appendix No. 2) and in Model Agreement I (Appendix No. 3) and Model Agreement II (Annex No. 3a) constituting an integral part of the SWZ. The contractual conditions for the performance of the public contract in question contain model contracts, which are an integral part of the SWZ. If there are grounds for entrusting the processing of personal data, the Contractor undertakes to sign a contract for entrusting the processing of personal data (in accordance with Annex 8 to the SWZ). Estimated value of the subject of the contract without the right of option PLN 1,466,353.00. In section II.1.5. announcements give an estimate of the total value taking into account the right of option.

The rules of the above-mentioned access are described in the model agreement, which is an integral part of the SWZ. For Task 4, the Contractor whose offer has been selected by the Contracting Authority as the most advantageous before the conclusion of the post-tender agreement will price the offered laparoscopic clipper, in accordance with the "List of leased device", which will constitute Annex 2 to the contract.
