1. The subject of the contract is the delivery and implementation of hardware infrastructure and software of the Hospital Information System (SSI) for the Municipal Hospital in Siemianowice Śląskie Sp. z o.o. 2. The contract is awarded as part of the project entitled "Improving the quality of medical services through the use of modern information and communication technologies in the Municipal Hospital in Siemianowice Śląskie Sp. z o.o." implemented from the Regional Operational Program of the Silesian Voivodeship for 2014-2020. 3. The order is carried out in two parts: Lot No. 1 – delivery and implementation of hardware infrastructure and software for the Hospital Information System (SSI). Part No. 2 – modernization of the LAN network in the scope of delivery and implementation of devices for wireless network.

A detailed description of the subject of the contract (OPZ) for each part of the contract is included in Annex 1.1 - 1.2 to the SWZ. All requirements specified in the OPZ are minimum requirements, and their fulfillment is obligatory. Failure to meet the above-mentioned minimum requirements will result in the rejection of the tender as inconsistent with the terms of the contract pursuant to Article 226(1)(5) of the PPL.
