Urban revitalization in Siemianowice Śląskie II

The subject of the contract is the renovation and modernization of existing multi-family residential buildings located at Matejki Street in Siemianowice Śląskie along with the modernization of residential premises and staircase and accompanying technical infrastructure, as well as revitalization of areas adjacent to the buildings. The subject of the order has been divided into four (4) lots.

Renovation and modernization of buildings located at 3, 4 and 14 Matejki Street in Siemianowice Śląskie - "design and build"

1. The subject of the order is:

1) development of comprehensive design documentation, hereinafter referred to as "design documentation" containing in particular: architectural and construction design together with land development design, technical and executive designs of all industries, cost estimate of works to be performed and Technical specifications for the execution and acceptance of works along with obtaining all necessary materials (including, m.in.: geodetic documentation and all necessary formal and legal documents, in including arrangements required by applicable law, recommendations, arrangements, permits, consents, decisions, including conservation permits / approval of the Municipal Conservator of Monuments and agreement/confirmation of sufficient connection of utilities or obtaining new technical conditions for connecting the power and gas networks, performing complementary measurements and checking the technical condition to the extent necessary to carry out the investment, etc.);

2) on the basis of design documentation, obtaining a legally valid decision on a building permit / notification of the intention to perform construction works;

3) on the basis of design documentation and a legally valid decision on a building permit or notification of the intention to perform construction works, to which no objection has been raised by way of a decision, the execution of construction works in the field of renovation and modernization of the building together with land development and all technical and accompanying infrastructure;

4) during the execution of construction works, performing author's supervision over their implementation;

5) preparation of acceptance documentation and as-built documentation, including as-built geodetic inventory, with submission of an as-built geodetic survey to the Department of Geodesy of the City of Siemianowice Śląskie and preparation of an energy performance certificate of the building and individual premises;

6) obtaining the necessary and required acceptance of the investment by all services and media administrators.

2. A detailed description of the subject of the contract for Lot 1 includes:

a) Functional and utility program (Appendix No. 3a to the SWZ),

b) Draft provisions of the agreement (Appendix No. 4a to the SWZ),

– hereinafter referred to as 'the Documentation',

3. The Contractor is obliged to perform the contract on the terms and conditions described in the Draft provisions of the contract (Appendix No. 4a to the SWZ).

4. For the subject of the contract, the selected Contractor shall provide the Contracting entity with a guarantee and warranty for a period consistent with the Contractor's offer, but not shorter than 60 months, counting from the day following the date of protocol acceptance without reservations of the entire subject of the contract.

5. The contracting authority has described the subject of the contract without indicating trademarks, patents or origin, source or specific process that characterizes the product or services provided by a specific contractor. If the contractor finds that the subject of the contract has been described in the manner referred to in Article 99(5) of the Act, i.e. the origin (brand, trademark, manufacturer, supplier) of the materials has been indicated, and the criteria used to assess equivalence have not been provided, it should ask the Contracting Authority to indicate what criteria will be used to assess equivalence.

6. If the documentation constituting a Detailed Description of the Subject of the Contract contains references to standards, technical assessments, technical specifications and technical reference systems referred to in Article 101(1)(2) and (3) of the PPL, the Contracting Authority shall allow solutions equivalent to those described. The use of a given standard, technical assessment, technical specifications and technical reference system in the documentation is only intended to clarify the level of expectations of the Ordering Party in relation to a specific material/device/product.

1. The subject of the order is:

1) development of comprehensive design documentation, hereinafter referred to as "design documentation" containing in particular: architectural and construction design together with land development design, technical and executive designs of all industries, cost estimate of works to be performed and Technical specifications for the execution and acceptance of works along with obtaining all necessary materials (including, m.in.: geodetic documentation and all necessary formal and legal documents, in including arrangements required by applicable law, recommendations, arrangements, permits, consents, decisions, including conservation permits / approval of the Municipal Conservator of Monuments and agreement/confirmation of sufficient connection of utilities or obtaining new technical conditions for connecting the power and gas networks, performing complementary measurements and checking the technical condition to the extent necessary to carry out the investment, etc.);

1. The subject of the order is:

1) development of comprehensive design documentation, hereinafter referred to as "design documentation" containing in particular: architectural and construction design together with land development design, technical and executive designs of all industries, cost estimate of works to be performed and Technical specifications for the execution and acceptance of works along with obtaining all necessary materials (including, m.in.: geodetic documentation and all necessary formal and legal documents, in including arrangements required by applicable law, recommendations, arrangements, permits, consents, decisions, including conservation permits / approval of the Municipal Conservator of Monuments and agreement/confirmation of sufficient connection of utilities or obtaining new technical conditions for connecting the power and gas networks, performing complementary measurements and checking the technical condition to the extent necessary to carry out the investment, etc.);
