The Services shall be made available to People with a Disability who are 16 or over and who are resident in York who are assessed as meeting the Care Act Eligibility Criteria and Children and Families Act 2014. This does not preclude other Individuals using the Services but funding for provision of Services shall be agreed with the person, their carer or other agencies / individuals prior to the provision of the Services.
Detailed elements within this specification are for:
a) Accommodation with support
b) Community Support, which includes (but not limited to):
i. Personalised support to enable access to chosen activities or to the general community
ii. Support at a base the Person has chosen to attend where different opportunities for Community activities are available
iii. Support to engage in work based training opportunities at a base
iv. Advice, guidance and signposting about resources in the community to meet individual outcomes
v. Volunteering, employment and peer support and Person led activities
vi. Personal and / or practical care to those that struggle to access their community independently
vii. Work based training and support to enable People to move into support or open employment
viii. Supported stimulating activities for People with profound and multiple disabilities
