2. Description of the object of the contract

The subject of the order is sludge loading with own equipment (loader), collection, transport and management by recovery or disposal of municipal sewage sludge in the installation (waste code 19 08 05) from the Group Sewage Treatment Plant in Kutno in the amount of approximately 68,000 tons.

The term of the contract: from 2.1.2019 to 31.5.2021.

Sludge collection will take place on dates agreed with the person appointed from the GOŚ.

Pursuant to the Act on waste from December 14, 2012 (Journal of Laws of 2018, item 992 of May 24, 2018), waste management is understood as collecting, transporting, processing waste, including supervision of this type activities, as well as subsequent handling of waste facilities and activities performed as a waste seller or waste broker.

3. Detailed description of the subject of the contract:

Technical and organizational data:

1) Average sludge hydration - 80% - 87%.

2) Way of stabilization: hygienization with quicklime, ground.

3) Before submitting the offer, the contractor must be familiar with the technical conditions for sludge collection. He must inspect the sludge loading site and obtain on his own responsibility and risk all information that may be necessary to prepare the offer and sign the contract for the task. Inspections will be made at the Contractor's expense.

4) The contractor should have a permit for waste collection and a waste treatment permit in accordance with art. 41 of the Act of 14.12.2012 on waste (Journal of Laws 2018, item 992 of 24.5.2018).

5) The Contractor should have an entry in the register (registration number) kept by the Marshal of the Voivodship, which allows conducting activities involving the transport of waste in accordance with art. 50 paragraph 1 point 5 lit. b of the Waste Act of December 14, 2012.

6) The Contractor will load sludge on cars with his own equipment (loader). The person operating the equipment should have the appropriate permissions. The Contractor bears full responsibility for the actions and omissions of the person operating the equipment (charger).

7) The contractor is obliged to manage waste in a manner consistent with the principles of sewage sludge management, in particular those set out in the Act of 14.12.2012 on waste (Journal of Laws of 2018, item 992), environmental protection requirements and plans waste management.

8) Upon the transfer of sewage sludge, the Recipient bears full legal and financial responsibility for collection, transport and management through recovery or disposal in the installation, in particular resulting from environmental protection regulations - Art. 27 of the Act of 14.12.2012 on waste (Journal of Laws of 2018, item 992).

9) The Contractor shall provide a description of the method of development (in accordance with Annex 5 - Description of the method of sewage sludge management) of the subject of the contract in accordance with the permits held. The method of developing the subject of the contract must be in accordance with applicable law.

10) The contractor should have at his disposal means of transport adapted to transport excessive sludge of the given hydration level.

11) The Employer has a car (stationary) scale. Each weighing of the sludge will be carried on the scale of the Employer (at the entrance and exit).

12) The basis for payment for the service will be documents confirming the weighing of the sludge. On the basis of weight receipts, the Employer will issue a collective waste transfer card once a month.

13) The Contractor is obliged to collect the sediment on the basis of the Employer's order sent by fax or email within 3 days from the date of notification.

14) The contractor should ensure systematic collection of sludge, also at temperatures below 0 about C.

15) Sewage sludge will be collected from the sewage treatment plant in Kutno on business days from 7:00 to 15:00.

16) At the end of each month, the contractor will present a summary statement of the amount of sludge taken into the installation (annex to the contract).

17) The Employer will provide the Contractor with the required sewage sludge test results for inspection.
